摘要
目的:探讨活血健脾止痛方联合隔物灸在股骨粗隆间骨折(IFF)术后的治疗效果。方法:将58例符合诊断标准行InterTan手术的患者作为研究对象,用随机数字表法进行分组,对照组、观察组各29例。对照组术后给予常规对症治疗,观察组在此基础上加活血健脾止痛方联合隔物灸治疗,比较两组术后中医证候积分、术后患肢肿胀程度、疼痛视觉模拟(VAS)、髋关节功能评分(HHS)。结果:治疗后,观察组总有效率为93.10%(27/29)、对照组为79.31%(23/29),观察组总有效率高于对照组(P<0.05)。干预前,对比两组中医证候积分,差异无统计学意义(均P>0.05)。治疗后观察组中医证候积分低于对照组,差异有统计学意义(均P<0.05)。治疗后第3、7天观察组患肢肿胀度低于对照组,差异有统计学意义(P<0.05)。治疗后第7、14天,观察组VAS评分、HHS评分均优于对照组(均P<0.05)。结论:活血健脾止痛方联合隔物灸能够调节IFF患者术后脾胃功能,减轻患肢疼痛、肿胀程度,有利于促进髋关节功能恢复。
Objective:To investigate clinical effect of Huoxue Jianpi Zhitong prescription combined with septum moxibustion in treatment of intertrochanteric fracture(IFF).Methods:58 patients who met diagnostic criteria and underwent InterTan surgery were selected as the research subjects regarded research subjects and randomly group using random number table method,29 cases in each group.The control group received conventional symptomatic treatment after the surgery,while the observation group were treated with combination of prescription for promoting blood circulation,strengthening spleen,alleviating pain,along with moxibustion with intervening substance on the basis.The improvement degrees of TCM syndrome scores,postoperative swelling of affected limb,visual analogue scale(VAS)of pain,and harris hip scring system(HHS)were compared between the two groups.Results:After treatment,the total effective rate of the observation group was 93.10%(27/29)higher than that in the control group 79.31%(23/29),difference statistically significant(P<0.05).Before treatment intervention,there was no significant difference in the TCM syndrome scores between the two groups(all P>0.05).After treatment,improvement in TCM syndrome scores in the observation group were more significant than those in the control group(all P<0.05).On the 3rd and 7th days after treatment,the degree of limb swelling in the observation group was significantly reduced compared with the control group(all P<0.05).On the 7th and 14th days after treatment,VAS scores and HHS scores of the observation group were better than those in the control group(all P<0.05).Conclusion:Huoxue Jianpi Zhitong prescription combined with septum moxibustion can regulate spleen and stomach function of patients after IFF surgery,reducing pain and swelling of affected limb.It was conducive to promoting recovery of hip joint function.
