摘要
目的:评估溶血磷脂酰胆碱酰基转移酶3(LPCAT3)对乙型肝炎病毒(HBV)相关肝细胞癌(HCC)患者的诊断价值。方法:收集2022年3月至2023年8月在武汉大学人民医院肿瘤科、传染科就诊的90例HBV相关性HCC患者(HBV-HCC组)、52例HBV感染后肝硬化患者(HBV-LC组)及体检中心49例健康体检者(对照组)的临床资料。比较各组人群血清LPCAT3及甲胎蛋白(AFP)水平,采用受试者工作特征曲线分析LPCAT3和AFP单独及联合检测对肝细胞癌的诊断价值。结果:对照组、HBV-LC组、HBV-HCC组血清LPCAT3和AFP水平依次增加,差异均有统计学意义(P<0.05)。HBV-HCC组与对照组进行鉴别诊断时,LPCAT3的曲线下面积(AUC)为1.000,敏感度和特异度均为100.00%;而AFP的AUC为0.842,敏感度65.60%,特异度100.00%;两者联合检测时AUC为1.000,特异度和灵敏度都为100.00%。HBV-HCC组与HBV-LC组鉴别诊断时,LPCAT3和AFP单独及联合检测的AUC分别为0.765、0.768和0.802。AFP阴性的HBV-HCC患者与其他组进行鉴别诊断,LPCAT3的AUC为0.831。结论:LPCAT3诊断HBV-HCC时其诊断价值强于AFP,且LPCAT3与AFP联合检测能提高HBV-HCC的诊断效率。
Objective:To evaluate the diagnostic value of lysophosphatidy lcholinacyltransferase 3(LPCAT3)in patients with hepatitis B virus-associated hepatocellular carcinoma.Methods:A total of 90 HBV-associated HCC patients,52 HBV-infected cirrhosis patients,and 49 healthy controls were collected from March 2022 to August 2023 in the Department of Oncology,Department of Infectious Disease and physical examination center of Wuhan University People′s Hospital.The levels of LPCAT3 and AFP were compared among all groups,and the diagnostic value of LPCAT3 and AFP alone or in combination was analyzed by using the subject work characteristic curve.Results:The levels of LPCAT3 and AFP in control group,HBV-LC group and HBV-HCC group were increased,with statistical significance(P<0.05).In the differential diagnosis between HBV-HCC group and control group,the area under the curve(AUC)of LPCAT3 was 1.000 with both sensitivity and specificity of 100.00%.AFP had an AUC of 0.842,with a sensitivity of 65.60%and a specificity of 100.00%.The AUC was 1.000 and the specificity and sensitivity were 100.00%.When the two were combined,the AUC was 1.000,and both specificity and sensitivity were 100.00%.In the differential diagnosis between the HBV-HCC group and the HBV-LC group,the AUC values for LPCAT3,AFP alone,and in combination were 0.765,0.768,and 0.802,respectively.For the differential diagnosis of HBV-HCC with AFP negative and other groups,the AUC of LPCAT3 was 0.831.Conclusion:LPCAT3 is more valuable than AFP in the diagnosis of HBV-associated hepatocellular carcinoma,and the combination of LPCAT3 and AFP can improve the diagnosis of HBV-associated hepatocellular carcinoma.
