摘要
目的系统评价麒麟丸联合手术对改善精索静脉曲张患者精子质量的效果。方法计算机检索中国知网、维普、万方、中国生物医学文献数据库、Cochrane Library、PubMed等数据库建库至2024年4月收录的麒麟丸联合手术治疗精索静脉曲张的中英文随机对照试验,进行文献筛选与阅读,采用Review Manager 5.4评价文献质量并绘制森林图。结果最终纳入资料完整的相关文献8篇,样本量715例,其中试验组383例,对照组332例。Meta分析结果显示,试验组(麒麟丸联合手术)在提升总有效率(OR=3.96,95%CI:2.49~6.29,P<0.00001)、提升精液量(MD=0.50,95%CI:0.44~0.56,P<0.00001)、提升精子浓度(MD=5.14,95%CI:0.44~0.56,P<0.00001)、提高a+b级精子活力(MD=9.39,95%CI:6.82~11.95,P<0.00001)、降低精子畸形比例(MD=-12.01,95%CI:-16.38~-7.64,P<0.00001)、提高配偶受孕率(OR=2.57,95%CI:1.64~4.02,P<0.00001)方面均优于对照组(单纯手术)。与对照组比较,试验组黄体生成素、卵泡刺激素水平均显著降低(P<0.05),总睾酮水平显著提高(P<0.05)。结论麒麟丸联合手术治疗精索静脉曲张患者可有效改善精子质量,提高配偶受孕率。
Objective To systematically evaluate the therapeutic effect of Qilin Pill combined with surgery on the sperm quality of patients with varicocele.Methods The databases of CNKI,VIP,Wanfang,China Biology Medicine,Cochrane Library and PubMed were searched by computer,and Chinese and English randomized controlled trials on Qilin Pill combined with surgery for the treatment of varicocele collected from the establishment of the database to April 2024 were included for literature screening and reading.Review Manager 5.4 was used for literature quality evaluation and forest map drawing.Results A total of 8 relevant literatures with complete data were included,with a sample size of 715 cases,including 383 cases in the experimental group and 332 cases in the control group.The results of Meta-analysis showed that the experimental group(Qilin Pill combined with surgery)was better than the control group(surgery only)in the aspects of improving the overall effective rate(OR=3.96,95%CI:2.49-6.29,P<0.000,01),increasing semen volume(MD=0.50,95%CI:0.44-0.56,P<0.000,01),increasing sperm concentration(MD=5.14,95%CI:0.44-0.56,P<0.000,01),increasing sperm motility of a+b(MD=9.39,95%CI:6.82-11.95,P<0.000,01),lowering the proportion of sperm malformation(MD=-12.01,95%CI:-16.38~-7.64,P<0.000,01),and improving the conception rate of spouses(OR=2.57,95%CI:1.64-4.02,P<0.000,01).Compared with the control group,the levels of luteinizing hormone and follicle stimulating hormone in the experimental group were significantly decreased(P<0.05),and the total testosterone level was significantly increased(P<0.05).Conclusions Qilin Pill combined with surgery can effectively improve the quality of sperm and increase the conception rate of spouses in treating patients with varicocele.
