摘要
目的探讨幽门螺杆菌(Helicobacter pylori,Hp)抗体分型检测对评估机体肝脏和贫血状态价值。方法选取淮安市第五人民医院154名健康体检人群为研究对象,检测其Hp抗体、血常规和常规生化指标,分析Hp抗体与各检测指标间的相关性。结果Hp抗体阴阳性与性别、年龄和体重指数无显著性相关(P>0.05)。Hp抗体阳性组(n=94)较阴性组(n=60),白细胞、中性粒细胞计数、淋巴细胞计数(lymphocyte count,LYM#)、血小板降低,总胆红素、直接胆红素、球蛋白升高,白球比下降(P<0.05)。Hp抗体Ⅰ型较Ⅱ型感染者的红细胞平均分布宽度变异系数高(P<0.05)。Hp抗体Ⅰ型不同分组比较结果显示,尿素酶抗体(urease antibodies,Ure Ab)(+)、空泡毒素相关蛋白A抗体(vacuotoxin associated protein A antibody,VacA Ab)(+)较Ure Ab(+)、VacA Ab(-)组红细胞平均体积(mean corpuscular volume,MCV)、平均血红蛋白量(mean corpuscular hemoglobin,MCH)、红细胞分布宽度标准差(standard deviation of red cell volume distribution width,RDW-SD)升高,LYM#降低(P<0.05);而Ure Ab(+)、细胞毒素相关蛋白A抗体(cytotoxin associated protein A antibody,CagA Ab)(+)、VacA Ab(+)较Ure Ab(+)、CagA Ab(+)、VacA Ab(-)组MCV、MCH、RDW-SD升高,丙氨酸氨基转移酶/天门冬氨酸氨基转移酶下降(P<0.05)。结论Hp抗体分型检测可评估机体肝脏和贫血状态,对于疾病的评估和治疗有指导作用。
Objective To explore the value of Helicobacter pylori(Hp)antibody typing in the assessment of liver and anemia status.Methods A total of 154 healthy people in the Fifth People's Hospital of Huai 'an were selected as the research objects to detect Hp antibody,blood routine and routine biochemical indexes,and analyze the correlation between Hp antibody and each detection index.Results There was no significant correlation with sex,age and body mass index between negative and positive Hp antibody group(P>0.05).The indexes of white blood cell count,neutrophil count,lymphocyte count(LYM#),platelet count and albumin-globulin ratio(A/G)with positive Hp antibody group(n=94)were significantly lower than the negative Hp antibody group(n=60),and the indexes of the total bilirubin,direct bilirubin,and globulin were contrary(P<0.05).The coefficient variation of red blood cell volume distribution width of Hp type Ⅰ infection was higher than the type Ⅱ infection(P<0.05).The comparison of different groups of Hp antibody type Ⅰ showed that the mean corpuscular volume(MCV)、mean corpuscular hemoglobin(MCH)and standard deviation of red blood cell volume distribution width(RDW-SD)of Urease antibodies(Ure Ab)(+)and Vacuotoxin associated protein A antibody,(VacA Ab)(+)groups were higher than Ure Ab(+)and VacA Ab(-)groups,and the LYM# was lower(P<0.05).Similarly,MCV,MCH and RDW-SD with Ure Ab(+),Cytotoxin associated protein A antibody,(CagA Ab)(+),VacA Ab(+)were higher than Ure Ab(+),CagA Ab(+),VacA Ab(-)group,yet the ratio of alanine aminotransferase with aspartate aminotransferase was decrease(P<0.05).Conclusion Hp antibody type can evaluate the liver and anemia status of the body,and has a guiding role in the evaluation and treatment of the disease.
