摘要
目的探讨基于光学相干断层扫描血管成像(OCTA)比较角膜塑形镜和离焦框架镜分别联合0.01%阿托品对青少年近视眼底微循环、黄斑形态的影响。方法选取2022年1月~2024年4月徐州市中医院门诊治疗的80例青少年近视患者,据随机数表法分为角膜塑形镜组(予角膜塑形镜联合0.01%阿托品治疗,n=40)、框架镜组(予离焦框架镜联合0.01%阿托品治疗,n=40),比较两组视力指标、眼部状态参数、眼底微循环、黄斑形态及不良反应。结果治疗后,框架镜组的裸眼视力较治疗前差异无统计学意义(P>0.05),而角膜塑形镜组的裸眼视力升高(P<0.05),角膜塑形镜组的眼轴增长量低于框架镜组(P<0.05);两组眼压比较差异无统计学意义(P>0.05),角膜塑形镜组泪膜脂质层厚度(LLT)、非圆指数(AI)、泪膜破裂时间(BUT)、角膜曲率低于框架镜组(P<0.05);角膜塑形镜组视网膜浅层血流密度(SVD)、脉络膜毛细血管血流密度(CCVD)、盘周放射状毛细血管血流密度(RPCD)、视网膜深层血流密度(DVD)高于框架镜组(P<0.05);角膜塑形镜组黄斑厚度(CMT)、中心凹下外层视网膜嵌合体带完整者占比、中心凹下脉络膜厚度(SFCT)、中心凹下内层视网膜厚度(IRT)高于框架镜组(P<0.05);两组不良反应发生率比较(17.50%vs.15.00%),差异无统计学意义(P>0.05)。结论基于OCTA,角膜塑形镜联合0.01%阿托品治疗青少年近视能延缓近视进展、改善眼底微循环、促进黄斑形态恢复。但与离焦框架镜相比,角膜塑形镜会降低泪膜稳定性。
Objective To investigate the effects of orthokeratology(Ok lens)and defocus frame lens combined with 0.01%atropine respectively on fundus microcirculation and macular morphology in adolescents with myopia based on optical coherence tomography angiography(OCTA).Methods A total of 80 adolescent myopia patients treated in the outpatient department of Xuzhou Traditiongal Chinese Medicine Hospital from January 2022 to April 2024 were selected and divided into the Ok lens group(treated with Ok lens combined with 0.01%atropine,n=40)and the frame lens group(treated with defocus frame lens combined with 0.01%atropine,n=40)according to random number table method.Visual acuity index,ocular state parameters,fundus microcirculation,macular morphology and adverse reactions were compared between the two groups.Results After treatment,there was no significant difference in the naked vision of the frame lens group compared with that before treatment(P>0.05),while the naked vision of the Ok lens group was increased(P<0.05),and the increase of the eye axis of the Ok lens group was lower than that of the frame lens group(P<0.05).There was no significant difference in intraocular pressure between the two groups(P>0.05).The tear film lipid layer thickness(LLT),A-circularity index(AI),tear film break-up time(BUT)and corneal curvature in the Ok lens group were lower than those in the frame lens group(P<0.05).The superficial vascular density(SVD),choroidal capillary vascular density(CCVD),radial peripapillary capillaries density(RPCD)and deep vascular density(DVD)in Ok lens group were higher than those in the frame lens group(P<0.05).The central macular thickness(CMT),the proportion of intact subfoveal outer retinal chimera,subfoveal choroid thickness(SFCT)and subfoveal inner retinal thickness(IRT)in the Ok lens group were higher than those in the frame lens group(P<0.05).The incidence of adverse reactions between the two groups(17.50%vs.15.00%)had no significant difference(P>0.05).Conclusion Based on OCTA,Ok lens combined with 0.01%atropine can delay the development of adolescent myopia,improve the fundus microcirculation,promote the recovery of macular morphology.But compared with defocus frame lens,Ok lens reduce tear film stability.
