摘要
目的:观察白内障患者植入融合景深延长和衍射多焦点技术的Tecnis Synergy人工晶状体(IOL)术后早期临床疗效。方法:回顾性临床研究。连续纳入2023年4—10月于武汉爱尔眼科医院汉口医院,行白内障摘除联合Tecnis Synergy IOL植入术的白内障患者18例(29眼),术后随访3个月。观察术后1 d、1周、1个月、3个月时的裸眼远距离视力(UDVA)、中距离视力(UIVA)、近距离视力(UNVA);术后3个月的波前像差、斯特列尔比值(SR)以及调制传递函数截止频率(MTF-cutoff)。采用中国版视功能指数量表(VF-14-CN)评价患者的视功能相关生活质量,并于术后3个月时进行离焦曲线检查以及脱镜率、光学干扰现象和满意度调查。采用配对t检验和重复测量方差分析对数据进行统计分析。结果:患者术前及各随访时间点之间UDVA差异有统计学意义(F=124.30,P<0.001),其中术后1 d、1周、1个月及3个月时的UDVA与术前UDVA相比均明显改善(均P<0.001),但术后1 d、1周、1个月及3个月各时间点之间的UDVA差异均无统计学意义(均P>0.05);术后1 d、1周、1个月及3个月各时间点之间的UIVA差异无统计学意义(F=1.47,P=0.239);术后1 d、1周、1个月及3个月各时间点之间的UNVA差异有统计学意义(F=7.97,P=0.002),且术后1周、1个月及3个月时的UNVA均较术后1 d提高(P=0.001、0.006、0.006)。术后3个月时,97%(28/29)、100%(29/29)、97%(28/29)的患者分别获得了0.1(LogMAR)及以上的UDVA、UIVA和UNVA。术后3个月全眼像差、彗差、三叶草像差均较术前明显降低(t=7.63、2.17、2.52,均P<0.05),SR值和MTF-cutoff数据均高于术前(t=-9.41、-10.61,均P<0.001)。离焦曲线结果显示在-2.50~+0.50 D屈光度范围内视力均为0.2(LogMAR)或更好。VF-14-CN问卷表得分为(98.1±1.1)分。术后远、中、近距离视物时患者脱镜率分别为100%、100%、86%。术后8眼(8/29)发生光学干扰现象。结论:白内障患者植入Tecnis Synergy IOL可获得良好连续的UDVA、UIVA和UNVA,且患者术后脱镜率高,满意度也高。
Objective:To observe early clinical outcomes of patients who underwent cataract surgery with the Tecnis Synergy intraocular lens(IOL)which combines extended depth-of-focus and diffractive multifocal profiles.Methods:This was a retrospective clinical study.A total of 18 patients(29 eyes)who underwent cataract extraction combined with Tecnis Synergy IOL implantation in Wuhan Aier Hankou Eye Hospital from April 2023 to October 2023 were consecutively included in this study.All participants were followed up for 3 months.Uncorrected distance,intermediate and near visual acuities(UDVA,UIVA and UNVA,respectively)were recorded postoperatively at 1 day,1 week,1 month and 3 months.Wavefront aberration,Strehl ratio(SR),and modulation transfer function cutoff frequency(MTF-cutoff)at 3 months after surgery were also recorded.Chinese version visual function index-14(VF-14-CN),defocus curve,spectacle independence,and patient satisfaction were assessed for visual performance in all participants.Data were analyzed using paired t-test and repeated measurement ANOVA.Results:There were significant differences in UDVA among before surgery and all follow-up time points postoperatively(F=124.30,P<0.001).The UDVA at all follow-up time points postoperatively were significantly improved compared with those before surgery(all P<0.001).But there were no significant differences in UDVA among all follow-up time points after the operation(all P>0.05).There were no significant differences in UIVA among all follow-up time points postoperatively(F=1.47,P=0.239).The differences of UNVA among different follow-up time points postoperatively were statistically significant(F=7.97,P=0.002),while UNVA at 1 week,1 month and 3 months after surgery was significantly improved than that at 1 day after surgery(P=0.001,0.006 and 0.006,respectively).A total of 97%(28/29),100%(29/29),and 97%(28/29)patients have shown UDVA,UIVA and UNVA of 0.1 logMAR or better respectively at 3 months after surgery.Total aberration,coma,and trefoil at 3 months after operation were significantly decreased(t=7.63,2.17 and 2.52,respectively,all P<0.05)compared to before operation.SR and MTF-cutoff after operation were significantly higher than those before operation(t=-9.41 and-10.61,both P<0.001).The defocus curve results showed that the visual acuities were 0.2 logMAR or better within the range of-2.50 D to+0.50 D.The score of the VF-14-CN questionnaire was 98.1±1.1.Spectacle independence of UDVA,UIVA and UNVA were 100%,100%and 86%respectively.The existence of photic phenomena occurred in 8 eyes.Conclusion:Implantation of the Tecnis Synergy IOL in cataract participants achieves favorable and consistent UDVA,UIVA and UNVA,leading to a high rate of spectacle independence and high patient satisfaction postoperatively.
