摘要
目的:观察祛浊生肌膏辅助治疗高位肛周脓肿术后的临床疗效。方法:选取2021年9月—2022年12月鹤壁煤业(集团)有限责任公司总医院收治的高位肛周脓肿患者92例,采用随机数字表法分为对照组、试验组各46例。2组均给予高位肛瘘挂线术治疗。对照组术后给予西药治疗,试验组在对照组基础上辅助祛浊生肌膏外敷治疗。比较2组治疗前后中医证候积分、视觉模拟评分法(VAS)评分、创面渗液评分、肛管最大收缩压、肛管静息压及血清白细胞介素-1β (IL-1β)、肿瘤坏死因子-α (TNF-α)、白细胞介素-8 (IL-8)、血管内皮生长因子(VEGF)水平;比较2组肉芽生长评分、创面愈合时间、临床疗效及不良反应发生情况。结果:试验组总有效率95.65%(44/46),高于对照组总有效率73.91%(34/46)。治疗后,2组中医证候积分、VAS评分、创面渗液评分、肛管最大收缩压、肛管静息压及血清TNF-α、IL-1β、IL-8水平均较治疗前降低(P<0.05),血清VEGF水平升高(P<0.05);试验组中医证候积分、VAS评分、创面渗液评分及各血清炎症因子水平低于对照组(P<0.05),肛管最大收缩压、肛管静息压及血清VEGF水平高于对照组(P<0.05)。试验组肉芽生长评分低于对照组(P<0.05),创面愈合时间短于对照组(P<0.05)。2组无不良反应发生。结论:祛浊生肌膏辅助治疗能够降低患者炎症反应,减轻创面疼痛,促进肉芽生长与肛门功能的恢复,疗效显著,安全性较高。
Objective:To observe the clinical effect of Quzhuo Shengji Ointment on prognosis after operation for high perianal abscess.Methods:A total of 92 patients after operation for high perianal abscess admitted to the General Hospital of Hebi Coal Industry(Group)Co.,Ltd.from September 2021 to December 2022 were selected and divided into the control group and the trial group by random number table method,with 46 patients in each group.The two groups were treated with high ligation therapy for anal fistula.The control group was treated with western medicine after operation,and the trial group was additionally treated with Quzhuo Shengji Ointment based on the treatment of the control group.The traditional Chinese medicine(TCM)syndrome scores,scores of Visual Analogue Scale(VAS),wound drainage score,anal canal maximum systolic blood pressure,anal canal resting pressure,serum levels of interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α),interleukin-8(IL-8)and vascular endothelial growth factor(VEGF)were compared between the two groups before and after treatment.The granulation growth score,wound healing time,clinical effects and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate was 95.65%(44/46)in the trial group,and was 73.91%(34/46)in the control group.After treatment,the TCM syndrome scores,VAS scores,wound draining scores,the anal canal maximum systolic blood pressure,anal canal resting pressure and levels of serum TNF-α,IL-1βand IL-8 were decreased in both groups when compared with those before treatment(P<0.05),and serum VEGF levels were increased(P<0.05);the TCM syndrome scores,VAS score,wound drainage score and levels of serum inflammatory factors in the trial group were lower than those in the control group(P<0.05),and the anal canal maximum systolic blood pressure,anal canal resting pressure and serum VEGF levels in the trial groups were higher than those in the control group(P<0.05).The granulation growth score in the trial group was lower than that in the control group(P<0.05),and the wound healing time was shorter than that in the control group(P<0.05).No adverse reactions occurred in both groups.Conclusion:The adjuvant treatment of Quzhuo Shengji Ointment can reduce the inflammatory responses of patients,relieve the wound pain,promote the growth of granulation and the recovery of anal function,with significant effect and high safety.
