摘要
目的规范无痛消化内镜检查麻醉流程,降低麻醉和检查风险,使无痛消化内镜检查的麻醉信息在临床科室、麻醉科、内镜室等部门之间全面准确地共享。方法梳理无痛消化内镜检查麻醉业务流程,确定医院信息系统、麻醉系统、消化内镜系统之间的数据交互方式,遵循医院信息平台交互规范,采用SOAP1.2通信协议、HL7 V3数据交互标准接口进行系统的设计与开发。比较系统使用前后无痛消化内镜检查患者的预约取消率、门诊患者检查预约耗时、住院患者检查前待床日的差异,验证改造方案的应用效果。结果系统使用后,无痛消化内镜检查患者的预约取消率由18.99%降低至7.57%,检查预约耗时由(174.49±55.71)min缩短至(139.82±45.10)min,检查前待床日由(2.36±1.68)d缩短至(2.05±1.46)d,且差异均有统计学意义(P<0.05)。结论通过信息系统改造实现无痛消化内镜患者的麻醉管理,可以保障无痛消化内镜患者的检查、麻醉安全,是提升患者就医体验、提高服务效率、改善医疗管理水平的有效方法和途径。
Objective To standardize the anesthesia process of painless digestive endoscopy,reduce the risk of anesthesia and examination,and share the anesthesia information of painless digestive endoscopy comprehensively and accurately among clinical departments,anesthesiology departments,endoscopy rooms and other departments.Methods The anesthesia business process of painless digestive endoscopy was sorted out,the data interaction mode among hospital information system,anesthesia system and digestive endoscopy system was determined.According to the interaction specification of hospital information platform,the system was designed and developed by using SOAP1.2 communication protocol and HL7 V3 data interaction standard interface.The differences before and after the use of the system in the cancellation rate of appointments for patients undergoing painless digestive endoscopy,the time spent on outpatient examination appointments,and the days of waiting for bed before inpatient examination were compared,so as to verify the application effect of the transformation plan.Results After the use of the system,the cancellation rate of appointments for patients undergoing painless digestive endoscopy decreased from 18.99%to 7.57%,the examination appointment time was shortened from(174.49±55.71)min to(139.82±45.10)min,and the days of waiting before examination was shortened from(2.36±1.68)d to(2.05±1.46)d,and all the differences were statistically significant(P<0.05).Conclusion Anesthesia management of patients with painless digestive endoscopy through information system transformation can ensure the safety of examination and anesthesia of patients with painless digestive endoscopy.It is an effective method and way to improve patients’medical experience,improve service efficiency and improve medical management level.
