疫情期间影响儿童口唇裂伤术后伤口感染的因素分析及诊疗探讨

摘要 目的:分析新冠疫情期间儿童口唇裂伤术后伤口感染的因素。方法:选取2021年7月至2022年11月福建省儿童医院收治入院行全身麻醉下急诊缝合的52例口唇裂伤患儿作为研究对象,统计其年龄、性别、伤口贯通、术前暴露时间(患儿意外发生后至手术开始的时间)、手术操作时间、营养状况、口唇裂伤长度、患儿心理情绪评分信息,分析患儿术后伤口感染的因素,并探讨术后诊疗方案。结果:52例口唇裂伤患儿术后伤口感染15例,经分析性别不是影响儿童口唇裂伤术后伤口感染因素,年龄、伤口贯通、术前暴露时间(患儿意外发生后至手术开始的时间)、手术操作时间、营养状况、口唇裂伤长度、患儿心理情绪是影响儿童口唇裂伤术后伤口感染因素,而年龄≤3岁、伤口贯通、术前暴露时间>12 h、手术操作时间>3 h、营养不良、口唇裂伤长度>3 cm、患儿心理情绪评分>52分为危险因素。结论:疫情期间,影响儿童口唇裂伤术后伤口感染的因素较为复杂,年龄≤3岁、伤口贯通、术前暴露时间>12 h、手术操作时间>3 h、营养不良、口唇裂伤长度>3 cm、患儿心理情绪评分>52分可增加儿童口唇裂伤术后发生感染概率,采取多方面的预防和干预措施,确保术后管理规范性,可规避相关风险,对降低感染可能性、保障儿童预后具有积极意义。 Objective To analyze the factors affecting postoperative wound infection in children with oral and lip lacerations during the corona virus disease 2019(COVID-19)pandemic.Methods A total of 52 children with oral and lip lacerations who underwent emergency suture under general anesthesia at Fujian Children's Hospital from July 2021 to November 2022 were selected as the research subjects.Their age,gender,wound penetration,preoperative exposure time(time from the accident to the start of surgery),operation duration,nutritional status,length of oral and lip lacerations,and children's psychological emotion scores were statistically analyzed.The factors contributing to postoperative wound infection in children were analyzed,and the postoperative diagnosis and treatment plans were discussed.Results Among the 52 children with oral and lip lacerations,and 15 cases of postoperative wound infections were observed.Gender was not found to be a factor affecting postoperative wound infection in children.Age,wound penetration,preoperative exposure time(time from the accident to the start of surgery),operation duration,nutritional status,length of oral and lip lacerations,and children's psychological emotion scores were factors affecting postoperative wound infection.The risk factors included age≤3 years,wound penetration,preoperative exposure time>12 h,operation duration>3 h,malnutrition,length of oral and lip lacerations>3 cm,and children's psychological emotion scores>52 points.Conclusion During the pandemic,the factors affecting postoperative wound infection in children with oral and lip lacerations are complex.Age≤3 years,wound penetration,preoperative exposure time>12 h,operation duration>3 h,malnutrition,length of oral and lip lacerations>3 cm,and children's psychological emotion scores>52 points can increase the risk of postoperative infection in children.Adopting comprehensive prevention and intervention measures and ensuring the standardization of postoperative management can avoid related risks,which is of positive significance in reducing the possibility of infection and ensuring the prognosis of children.
