摘要
目的:探讨十一味参芪片联合甲巯咪唑治疗甲状腺功能亢进症合并白细胞减少患者的临床效果。方法:选取三明市第一医院2022年1月到2023年1月收治的88例甲状腺功能亢进症合并白细胞减少患者为研究对象,采取抽签法进行分组,分为观察组与对照组,每组各44例。对照组应用甲巯咪唑治疗,观察组应用十一味参芪片联合甲巯咪唑治疗,比较两组患者的临床疗效,治疗前后中医证候积分、白细胞计数以及相关甲状腺激素水平变化。结果:观察组患者治疗总有效率为95.45 %,高于对照组的77.27 %,差异具有统计学意义(P<0.05)。治疗后,观察组患者手指震颤、甲状腺肿、少寐多梦、急躁易怒、心悸不宁、纳呆、倦怠乏力积分均低于对照组,差异具有统计学意义(P<0.05)。治疗1周后、1个月后、3个月后,观察组患者外周血白细胞计数均高于对照组,差异具有统计学意义(P<0.05)。治疗后观察组患者血清促甲状腺激素(TSH)水平高于对照组,游离甲状腺激素(FT4)、游离三碘甲状腺原氨酸(FT3)水平低于对照组,差异具有统计学意义(P<0.05)。结论:甲状腺功能亢进症合并白细胞减少患者在甲巯咪唑基础上增加十一味参芪片治疗可提升其临床疗效,降低中医证候积分,增加白细胞计数,提升患者甲状腺功能。
Objective To explore the clinical efficacy of Eleven Flavor Shenqi tablets combined with methimazole in the treatment of patients with hyperthyroidism and leukopenia.Methods A total of 88 patients with hyperthyroidism and leukopenia admitted to Sanming First Hospital from January 2022 to January 2023 were selected as the research subjects.The study was divided into an observation group and a control group by drawing lots,with 44 patients in each group.The control group was treated with methimazole,while the observation group was treated with a combination of Eleven Flavor Shenqi tablets and methimazole.The clinical efficacy,traditional Chinese medicine syndrome scores,white blood cell count,and related thyroid hormone levels before and after treatment were compared.Results The total effective rate of treatment in the observation group was 95.45%,which was higher than 77.27%in the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of finger tremor,goiter,less sleep and more dreams,irritability,palpitation,lethargy and fatigue in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The white blood cell count of the observation group patients was higher than that of the control group after 1 week,1 month,and 3 months of treatment,and the difference was statistically significant(P<0.05).After treatment,the level of serum thyroid stimulating hormone(TSH)in the observation group was higher than that in the control group,while the levels of serum free thyroid hormone(FT4)and free triiodothyronine(FT3)were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Patients with hyperthyroidism complicated with leukopenia treated with Eleven Flavor Shenqi tablets on the basis of methimazole can improve their clinical efficacy,reduce traditional Chinese medicine syndrome scores,increase white blood cell counts,and enhance thyroid function.
