摘要
目的探讨分析纤维支气管镜灌洗+血必净治疗重症肺炎并急性呼吸窘迫综合征的临床效果。方法选取2022年8月至2023年8月期间云南省楚雄彝族自治州中医医院收治的66例重症肺炎并急性呼吸窘迫综合征患者,按入院先后划分为对照组与观察组,各33例。对照组采用血必净治疗,观察组采用纤维支气管镜灌洗联合血必净治疗。对比两组总体疗效。结果相比对照组,观察组总有效率、临床生存率均更高,呼吸机脱机时间与机械通气时间均更短,P<0.05。经过治疗,两组PaO2、HCO_(3)^(-)、pH值、PaO_(2)/FiO_(2)均大于治疗前,且观察组均大于对照组,P<0.05,血清PCT、CRP、IL-8及TNF-α水平均低于对照组,P<0.05。结论纤维支气管镜灌洗联合血必净治疗重症肺炎并急性呼吸窘迫综合征患者效果确切,可改善血气分析,缓解炎症反应,缩短机械通气时间,提高患者生存率。
Objective:To explore and analyze the clinical effect of fiberoptic bronchoscopy lavage plus Xuebijing in the treatment of severe pneumonia complicated with acute respiratory distress syndrome.Methods:From August 2022 to August 2023,66 patients with severe pneumonia complicated with acute respiratory distress syndrome in weichu Hospital of Traditional Chinese Medicine were selected and divided into control group and observation group,with 33 cases in each group.The control group was treated with Xuebijing,while the observation group was treated with fiberoptic bronchoscope lavage combined with Xuebijing.The overall curative effect of the two groups was compared.Results:Compared with the control group,the total effective rate and clinical survival rate of the observation group were higher,and the ventilator offline time and mechanical ventilation time were shorter,P<0.05.After treatment,the PaO2、HCO_(3)^(-)、pH value and PaO_(2)/FiO_(2)in both groups were higher than before treatment,and the observation group was higher than the control group,and the serum levels of PCT、CRP、IL-8 and TNF-αwere lower than those in the control group,P<0.05.Conclusion:Bronchofiberscope lavage combined with Xuebijing is effective in the treatment of severe pneumonia complicated with acute respiratory distress syndrome,which can improve blood gas analysis,alleviate inflammatory reaction,shorten mechanical ventilation time and improve the survival rate of patients.
