摘要
目的对比加温湿化高流量鼻导管氧疗(HFNC)与无创正压通气(NIPPV)治疗急性Ⅰ型呼吸衰竭(ARI)患者的临床效果。方法选择2021年6月至2023年6月扬州大学附属医院EICU收治的68例呼吸衰竭患者,采用随机数表法分为常规组与实验组,各34例。常规组应用NIPPV治疗,实验组应用HFNC。对比两组生命体征、呼吸困难程度。结果实验组治疗后心率、呼吸频率、血氧分压等指标均优于常规组,P<0.05;治疗后1周、2周、3周,实验组呼吸困难评分均明显低于常规组,P<0.05。结论相较于NIPPV治疗,HFNC对急性Ⅰ型呼吸衰竭患者的疗效更加显著,可稳定患者生命体征,改善呼吸困难症状。
Objective:This study aims to evaluate and contrast the clinical outcomes of heated humidified high-flow nasal cannula(HHFNC)therapy versus non-invasive positive pressure ventilation(NIPPV)in patients with acute type I respiratory failure.Methods:A total of 68 patients diagnosed with respiratory failure were enrolled from the EICU at Yangzhou University Affiliated Hospital between June 2021 and June 2023.These patients were randomly assigned to either the routine care group(34 patients receiving NIPPV)or the experimental group(34 patients treated with HHFNC).The study focused on comparing vital signs and the severity of dyspnea between the two groups.Results:Post-treatment,the experimental group showed significant improvements in heart rate,respiratory rate,and arterial oxygen partial pressure compared to the routine group,P<0.05.Additionally,over the course of 1、2 and 3 weeks,the dyspnea scores in the experimental group were markedly lower than those in the routine group,P<0.05.Conclusion:Compared with non-invasive positive pressure ventilation,heating and humidifying high-flow nasal catheter oxygen therapy is more effective in patients with acute type I respiratory failure,which can stabilize the vital signs of patients and improve the symptoms of dyspnea.
