摘要
目的 观察腹针引气归元法联合透刺电针对急性期周围性面瘫患者Portmann评分、神经营养因子的影响。方法 选取2020年1月—2023年6月期间80例周围性面瘫急性期患者作为研究对象,采用随机数字表法分为观察组、对照组各40例,对照组予以基础治疗配合透刺电针治疗,观察组在对照组的基础上联合腹针引气归元法治疗,连续治疗4周。比较两组临床疗效,治疗前后急性伴随症状积分、Portmann评分、血清神经营养因子水平。结果 观察组总显效率和痊愈率均高于对照组(P<0.05);治疗7 d时,两组耳痛、面部肿痛、颈强不适、恶寒发热症状积分均较治疗前降低(P<0.05),且观察组低于对照组(P<0.05);治疗4周后,两组Portmann评分皱眉、闭眼、张大鼻孔、吹口哨、微笑、鼓腮均较治疗前提高(P<0.05),且观察组高于对照组(P<0.05);治疗4周后,两组血清神经生长因子(nerve growth factor, NGF)、胶质细胞源性神经营养因子(glial cell-derived neurotrophic factor, GDNF)水平均较治疗前提高(P<0.05),且观察组高于对照组(P<0.05)。结论 腹针引气归元法联合透刺电针可显著提高急性期周围性面瘫患者的治疗效果,有效缓解伴随症状,提高Portmann评分,促进神经营养因子的释放。
Objective To observe the effect of abdominal acupuncture combined with transcutaneous electrical acupuncture on the Portmann score and neurotrophic factors in patients with acute peripheral facial paralysis.Methods Eighty patients with acute peripheral facial paralysis from January 2020 to June 2023 were selected as research subjects.Using a random number table method,they were divided into observation and control groups,each with 40 cases.The control group received basic treatment combined with transcutaneous electrical acupuncture,while the observation group received abdominal acupuncture on the basis of the control group,continuously for 4 weeks.The clinical efficacy,acute accompanying symptom scores,Portmann scores and serum levels of neurotrophic factors were compared between the two groups.Results The overall effective rate and cure rate of the observation group were significantly higher than those of the control group(P<0.05).After 7 days of treatment,the scores of ear pain,facial swelling,neck discomfort,and chills and fever in both groups were significantly reduced compared to those before treatment(P<0.05),and the levels in the observation group were lower than those of the control group(P<0.05).After 4 weeks of treatment,the Portmann scores for frowning,closing eyes,flaring nostrils,whistling,smiling and puffing cheeks in both groups were significantly improved compared to those before treatment(P<0.05),and the scores of the observation group were higher than those of the control group(P<0.05).After 4 weeks of treatment,the serum levels of nerve growth factor(NGF)and glial cell-derived neurotrophic factor(GDNF)in both groups were significantly increased compared to those before treatment(P<0.05),and the levels in the observation group were higher than those in the control group(P<0.05).Conclusion Abdominal acupuncture combined with transcutaneous electrical acupuncture can significantly improve the treatment effect in patients with acute peripheral facial paralysis,effectively alleviate accompanying symptoms,improve the Portmann score and promote the release of neurotrophic factors.
