摘要
角药即3味中药的联合应用,是临床中配伍用药的特殊形式。根据中药性味归经及七情和合等原则同时应用3味中药,可发挥相互促进、相反相成、减毒增效的作用。在疾病诊治过程中针对患者不同的临床表现及病因病机联合应用多组角药,常获事半功倍之效。谢晶日教授多年从事临床工作,尤擅诊治消化系统疾病,对慢性萎缩性胃炎(chronic atrophic gastritis, CAG)的诊治颇有心得,认为CAG的病机多属湿阻、湿热、肝郁、血瘀、阴亏、脾胃虚寒,总属本虚标实、虚实夹杂,基本病机为脾胃虚弱。在治疗上,湿阻者应芳香化湿,湿热者宜清利湿热,肝郁者可疏肝理脾,血瘀者宜行气活血,阴亏者应滋养胃阴,虚寒者应温脾散寒,同时将宏观辨病、微观辨证贯穿其中,提高临床疗效,改善患者的生存质量,预防癌变。现将谢晶日教授治疗慢性萎缩性胃炎常用的6组角药及用药心得总结归纳,希望为中医药治疗慢性胃炎萎缩性提供新的思路和方法。
Triangular medicine,the combined application of three traditional Chinese medicines,is a special form of compati ble drug use in clinical practice.Based on the principles of meridian tropism of Chinese drugs and seven emotions in harmony,the simultaneous application of the three Chinese drugs can promote each other,reverse each other,reduce the toxicity and increase the efficiency.In the process of diagnosis and treatment,the combination of multiple groups of triangular medicine is used aiming at different clinical manifestations,etiology and pathogenesis,and often obtains the effect of getting twice the result with half the effort.Professor XIE Jingri has been engaged in clinical work for many years,especially is good at the diagnosis and treatment of digestive system diseases,and has good experience in the diagnosis and treatment of chronic atrophic gastritis.He believes that the pathogenesis of chronic atrophic gastritis mostly belongs to dampness obstruction,damp-heat,liver stagnation,blood stasis,Yin deficiency and deficiency cold of spleen and stomach,and it always belongs to the type of deficiency and excess,deficiency and excess in complexity.The basic pathogenesis is the deficiency of spleen and stomach.In terms of treatment,those with dampness obstruction should be treated with aromatic drugs to remove dampness,those with damp-heat should be treated with the method of clearing away and removing the dampness-heat,those with liver stagnation should be treated with the method of soothing liver and regulating spleen,those with blood stasis should be treated with the method of moving Qi and activating blood,those with Yin deficiency should be treated with the method of nourishing stomach Yin,and those with deficiency cold should be treated with the method of warming spleen and dissipating cold.Macroscopic and microscopic syndrome differentiation methods are integrated into the treatment to improve the clinical efficacy,the quality of life and prevent canceration.It summarized the commonly used six groups of triangular medicine and their application experience in the treatment of chronic atrophic gastritis by Professor XIE Jingri,in the hope of providing new ideas and methods for the treatment of chronic atrophic gastritis with traditional Chinese medicine.
