摘要
目的:分析腹腔镜与开腹手术治疗胆总管结石的临床效果。方法:选取2021年4月—2023年4月盘州市人民医院收治的60例胆总管结石患者,随机分为传统组(行开腹手术)和观察组(行腹腔镜手术),各30例。比较两组手术状况、术后恢复情况及患者心理状态与生活质量。结果:观察组手术时间、切口长度短于传统组,术中出血量少于传统组(P<0.001);观察组术后首次排气时间、肠胃功能恢复时间、住院时间短于传统组,术后视觉模拟评分法评分低于传统组(P<0.001);干预后,两组躯体、心理、症状、社会功能评分及总分均升高,观察组高于传统组(P<0.05)。结论:腹腔镜手术在胆总管结石治疗中的效果优于开腹手术,具有术后疼痛轻、恢复快、住院时间短、术中出血量少、切口小等优势,可有效改善患者心理状态及生活质量。
Objective:To analyze the clinical effect of laparoscopic and open surgery for choledocholithiasis.Methods:Sixty patients with choledocholithiasis admitted to Panzhou People's Hospital from April 2021 to April 2023 were randomly divided into traditional group(open surgery)and observation group(laparoscopic surgery),with thirty patients in each group.The operation status,postoperative recovery,psychological state and quality of life were compared between the two groups.Results:The operation time and incision length of the observation group were shorter than that of the traditional group,and the intraoperative blood loss was less than that of the traditional group(P<0.001).The first postoperative exhaust time,gastrointestinal function recovery time and hospitalization time of the observation group were shorter than those of the traditional group,and the score of the visual analog score was lower than that of the traditional group(P<0.001).After intervention,the scores of body,psychology,symptoms,social function and total scores of both groups were increased,and the observation group was higher than the traditional group(P<0.05).Conclusion:The effect of laparoscopic surgery in the treatment of choledocholithiasis is better than that of open surgery,which has the advantages of light postoperative pain,quick recovery,short hospital stay,less intraoperative blood loss,small incision,etc.,and can effectively improve the psychological state and quality of life of patients.
