摘要
目的:分析达格列净联合二甲双胍治疗血糖控制不佳的2型糖尿病(T2DM)伴肥胖的效果。方法:选取2022年7月—2024年4月东营市第五人民医院收治的70例血糖控制不佳的T2DM伴肥胖患者,根据随机数字表法分为对照组(n=35,采取二甲双胍治疗)和观察组(n=35,在对照组基础上联合达格列净治疗)。比较两组患者血糖指标、肥胖情况、胰岛素情况及血脂水平。结果:治疗后,两组糖化血红蛋白、空腹血糖及餐后2 h血糖水平降低,观察组低于对照组(P<0.05);治疗后,两组体质量指数、腰围均下降,观察组低于对照组(P<0.05);治疗后,两组空腹胰岛素、餐后2 h胰岛素及基础胰岛素分泌功能指数均升高,观察组高于对照组(P<0.05);治疗后,两组总胆固醇、甘油三酯、低密度脂蛋白胆固醇水平降低,高密度脂蛋白胆固醇水平升高,观察组优于对照组(P<0.05)。结论:应用达格列净联合二甲双胍治疗血糖控制不佳的T2DM伴肥胖,能够降低患者血糖水平,改善胰岛素分泌能力、肥胖情况及血脂水平。
Objective:To analyze the effect of dapagliflozin combined with metformin in the treatment with type 2 diabetes(T2DM)and obesity with poor blood glucose control.Methods:A total of 70 patients with T2DM and obesity with poor blood glucose control admitted to Dongying Fifth People's Hospital from July 2022 to April 2024 were selected and divided into a control group(n=35,treated with metformin)and an observation group(n=35,treated with dapagliflozin combined with the control group)according to the random number table method.The blood glucose indexes,obesity,insulin status and blood lipid levels of the two groups were compared.Results:After treatment,the levels of glycosylated hemoglobin,fasting blood glucose and 2 h postprandial blood glucose in both groups decreased,and the observation group was lower than that in the control group(P<0.05).After treatment,the body mass index and waist circumference of both groups decreased,and the observation group was lower than that in the control group(P<0.05).After treatment,the fasting insulin,2 h postprandial insulin and basal insulin secretion function index of both groups increased,and the observation group was higher than that in the control group(P<0.05).After treatment,the levels of total cholesterol,triglycerides and low-density lipoprotein cholesterol decreased,and the level of high-density lipoprotein cholesterol increased in both groups,and the observation group was better than the control group(P<0.05).Conclusion:The application of dapagliflozin combined with metformin in the treatment of T2DM with poor blood glucose control and obesity can reduce the blood glucose level of patients,improve insulin secretion ability,obesity and blood lipid levels.
