摘要
胃泌素是一种由胃窦G细胞分泌的多肽类激素,主要生理功能是促进胃酸分泌和细胞增殖,目前胃泌素联合胃蛋白酶原、幽门螺杆菌在临床上已被广泛用于胃癌癌前病变及早期胃癌的筛查。此外,胃泌素水平过低可导致胃酸分泌不足,从而引起消化不良和胃内感染;而胃泌素水平过高又是胃腺瘤发生的重要危险因素。因此,胃泌素水平对临床早癌筛查以及人体消化功能十分重要。目前有研究发现,多种临床常用药物均可显著影响胃泌素的水平,包括质子泵抑制剂、钾竞争性酸阻滞剂、生长抑素及其类似物、胃泌素受体拮抗剂等。因此,在临床中对胃泌素水平的分析需排除上述药物的影响,同时为了避免高胃泌素血症或胃泌素缺乏的发生,应避免相关药物的滥用,并采取合适药物调整胃泌素水平。本文对影响胃泌素水平的相关药物进行了综述。
Gastrin is a polypeptide hormone secreted by G cells in the gastric antrum,and its main physiological functions are to promote gastric acid secretion and cell proliferation.Currently,gastrin,combined with pepsinogen and Helicobacter pylori,has been widely used in the clinic for the screening of precancerous lesions and early gastric cancer.In addition,hypogastrinemia can lead to insufficient gastric acid secretion,resulting in indigestion and gastric infections;however,hypergastrinemia is an important risk factor for the development of gastric adenoma.Therefore,gastrin levels are very important for clinical screening of early cancer and digestive function.It has been found that many commonly used drugs in clinical practice can significantly affect gastrin levels,including proton pump inhibitors,potassium-competitive acid blockers,somatostatin and its analogs,and gastrin receptor antagonists.Therefore,in clinical practice,the analysis of gastrin levels needs to exclude the influence of the above drugs.At the same time,the abuse of related drugs should be avoided and appropriate drugs should be used to adjust gastrin levels in order to avoid the occurrence of hypergastrinemia or gastrin deficiency.This article reviews the related drugs that affect the level of gastrin.
