摘要
“督脉入脑”理论最早见于《黄帝内经》《难经》等古籍中,后经张锡纯等医家再加以诠释,该理论由此形成。近年来,随着卒中发病率不断上升,卒中后吞咽障碍(PSD)的患病率逐渐升高。中医学认为,PSD可归属于“中风”“喑痱”“喉痹”等范畴。作者基于“督脉入脑”理论从肾精不足、阳脉虚衰、瘀血不去3方面分别论述PSD的病因、病机及治疗思路,针对其肾精不足之病因病机,可用补肾益髓开窍汤;针对其阳脉虚衰之病因病机,可用通督调神针法;针对其瘀血不去之病因病机,可用通督化瘀灸。从“督脉入脑”理论论治PSD体现出中医学辨证论治思想,能够为PSD的治疗提供新的方法与思路。
The concept of the"governor's vein entering the brain"was initially documented in ancient texts such as"Huangdi's Inner Classic"and"The Difficulties,"later elucidated by medical practitioners like Zhang Xichun,thereby giving rise to the theory.In recent years,with the escalating incidence of stroke,the incidence of post-stroke dysphagia(PSD)has progressively risen.Traditional Chinese Medicine posits that dysphagia following stroke can be categorized under"stroke,""paralysis,""throat obstruction,"among other classifications.This article explores the etiology,pathogenesis,and treatment modalities of PSD through the lens of three principles:kidney essence insufficiency,Yang pulse debility,and failure to resolve blood stasis,all based on the theoretical framework of"Du pulse entering the brain."Drawing from the etiology and pathogenesis of Yang pulse debility,the formula Tongdu Tiao Shenpin may be employed.In light of the etiology and pathogenesis of blood stasis,therapies such as blood-activating moxibustion can be implemented.Approach to the management of PSD from the perspective of"guiding pulse into the brain"embodies the diagnostic principles of TCM,offering novel therapeutic approaches and insights for the management of PSD.
