摘要
曹守沛教授认为,“脾虚内伤,痰浊、瘀血内生痹阻心脉”是冠状动脉粥样硬化性心脏病(简称冠心病)发生的常见病因,“心脾气虚,痰瘀阻络”为其常见病机。曹教授注重“心脾同治”,倡导从脾胃论治冠心病,以健脾益气、化痰通络为主要治疗原则,将“健脾益气通脉”的思想贯穿冠心病治疗的始终,自拟“调脾通络方”加减辨治冠心病。曹教授治疗过程中,药物用量均衡,标本兼顾,痰瘀并治,同时衷中参西,对“证”亦对“病”、对“症”,分别施以针对性用药,实现全面辨治,整体调护。气虚血瘀者,因其痰浊证素不显,兼有血虚之候,以自拟方合桃红四物汤加减治之;痰浊、血瘀并见者则以自拟方合枳实薤白桂枝汤或二陈汤加减,以痰瘀同治。
Professor Cao Shoupei believes that"internal injury due to spleen deficiency,with the generation of phlegm turbidity and blood stasis blocking the heart meridians"is a common cause of coronary atherosclerotic heart disease(CHD),and"heart-spleen qi deficiency,phlegm and blood stasis blocking the meridians"is a common pathogenesis of it.Professor Cao emphasizes the"co-treatment of the heart and spleen",advocating the treatment of CHD from the perspective of the spleen and stomach.He takes invigorating the spleen,supplementing qi,resolving phlegm and unblocking the meridians"as the main therapeutic principles,and consistently applies the idea of"invigorating the spleen,supplementing qi and unblocking the meridians"throughout the treatment of CHD.He has developed a self-designed Spleen-Regulating and Meridian-Unblocking Decoction for syndrome differentiation and treatment of CHD.During the treatment process,Professor Cao pays attention to balancing dosage of medication,addressing both the symptoms and the root cause,treating phlegm and blood stasis simultaneously.He also integrated traditional Chinese medicine with Western medicine,treating both the"syndrome"and the"disease",as well as the"symptoms",and administers targeted medications respectively to achieve comprehensive diagnosis and treatment,as well as overall care.For those with qi deficiency and blood stasis,as their phlegm turbidity syndrome factors are not obvious and they also have symptoms of blood deficiency,a self-designed formula combined with Modified Peach Kernel and Carthamus Four Substances Decoction is used.For those with concurrent phlegm turbidity and blood stasis,a self-designed formula combined with Modified Immature Bitter Orange and Chinese Chive and Cinnamon Twig Decoction or Two Matured Substances Decoction is used to treat both phlegm and stasis simultaneously.
