摘要
目的 分析维持性血液透析患者出现低血压情况的影响因素。方法 回顾性分析我院2022年8月至2023年5月收治的130例维持性血液透析患者的临床资料,根据是否出现低血压将其分为发生组(45例)、未发生组(85例)。比较两组的一般资料、生化指标及超滤率、透析时间。结果 两组的性别、原发性疾病占比比较,差异无统计学意义(P>0.05);发生组的年龄大于未发生组,合并高血压、高血脂、高血糖占比高于未发生组(P<0.05)。两组的血肌酐(Scr)、尿酸(UA)、血尿素氮(BUN)、血钠、血钙水平比较,差异无统计学意义(P>0.05);发生组的白蛋白(ALB)水平低于未发生组,低密度脂蛋白胆固醇(LDL-C)、N末端B型钠尿肽原(NT-proBNP)水平、超滤率高于未发生组,透析时间长于未发生组(P<0.05)。结论 高龄、超滤率高、透析时间长、低ALB水平及高LDL-C、NT-proBNP水平使得维持性血液透析患者更易出现低血压,临床可据此制定科学的干预措施,从而保障患者预后。
Objective To analyze the influencing factors of hypotension in maintenance hemodialysis patients.Methods The clinical data of 130 patients with maintenance hemodialysis admitted in our hospital from August 2022 to May 2023 were retrospectively analyzed.According to whether hypotension occurred,the patients were divided into occurrence group(45 cases)and non-occurrence group(85 cases).The general data,biochemical indexes,ultrafiltration rate and dialysis time were compared between the two groups.Results There were no significant differences in gender and proportion of primary diseases between the two groups(P>0.05);the age in the occurrence group was bigger than that in the non-occurrence group,and the proportions of hypertension,hyperlipidemia and hyperglycemia were higher than those in the non-occurrence group(P<0.05).There were no significant differences in serum creatinine(Scr),uric acid(UA),blood urea nitrogen(BUN),serum sodium and serum calcium levels between the two groups(P>0.05);the albumin(ALB)level in the occurrence group was lower than that in the non-occurrence group,the levels of low density lipoprotein cholesterol(LDL-C),N-terminal pro-B-type natriuretic pepfides(NT-proBNP)and ultrafiltration rate were higher than those in the non-occurrence group,and dialysis time was longer than that in the non-occurrence group(P<0.05).Conclusion Advanced age,high ultrafiltration rate,long dialysis time,low ALB level and high levels of LDL-C and NT-proBNP make maintenance hemodialysis patients more prone to hypotension.Clinically,scientific intervention measures can be formulated accordingly to ensure the prognosis of patients.
