摘要
目的 探究精神分裂症患者血清炎性细胞因子水平与临床症状及疗效的相关性。方法 纳入2021年12月~2023年5月在首都医科大学附属北京安定医院芜湖医院住院的首发或停药4周以上的精神分裂症患者50例作为研究组,选择同时期在本院体检的健康志愿者50例作为对照组,在使用奥氮平治疗6周前后检测血清白细胞介素(Interleukin,IL)-2、IL-4、IL-8、IL-10、肿瘤坏死因子-α(Tumour Necrosis Factor-α,TNF-α)的浓度,使用阳性与阴性症状量表(positive and negative Symptom scales,PANSS)对精神病症状严重程度进行评估。比较治疗前后细胞因子水平、PANSS评分的变化,分析炎性细胞因子基线水平与PNASS评分及差值(治疗后-治疗前)的相关性,并使用多元线性回归分析探索炎性细胞因子基线水平与PANSS评分差值之间的关系。结果 治疗前研究组血清IL-10、TNF-α水平较对照组高,血清IL-8水平较低。治疗6周后患者血清IL-8浓度升高,IL-10、TNF-α浓度降低。治疗前IL-2、TNF-α与PANSS评分负相关,IL-10与PANSS评分正相关。TNF-α基线水平与阴性症状评分差值正相关。多元线性回归确定TNF-α基线水平与阴性症状评分差值显著正相关(调整R方=0.198,β=0.445,P=0.001)。结论 精神分裂症患者血清炎性细胞因子水平与临床症状有一定相关性,奥氮平可调节异常炎性细胞因子水平,TNF-α可以预测奥氮平对阴性症状的疗效。
Objective To examine the correlation between serum inflammatory cytokines and clinical symptoms/efficacy in schizophrenia patients.Methods 50 schizophrenia patient hospitalised in Wuhu Hospital of Beijing Anding Hospital affiliated to Capital Medical Universitys(newly admitted or off medication >4 weeks)and 50 healthy controls were recruited.Serum IL-2,IL-4,IL-8,IL-10,TNF-α were measured before and after 6 weeks of olanzapine treatment.PANSS assessed symptoms.Changes in cytokines and PANSS scores were compared.Correlation between baseline cytokines,PANSS scores,and treatment differences was analyzed using multiple linear regression.Results Pre-treatment,patients had higher IL-10,TNF-α,lower IL-8.Post-treatment,IL-8 increased,IL-10,TNF-αdecreased.IL-2,TNF-α negatively correlated with PANSS;IL-10 positively correlated.TNF-α baseline positively correlated with negative symptom score difference.Multiple linear regression showed TNF-α baseline predicted negative symptom score difference(adjusted R~2=0.198,β=0.445,P=0.001).Conclusion In schizophrenia patients,serum inflammatory cytokine levels correlate with symptoms.Olanzapine modulates these cytokines,with TNF-α potentially predicting its efficacy on negative symptoms.
