摘要
目的探讨加味瓜蒌散联合毫火针预防带状疱疹后遗神经痛(PHN)的效果,以期为临床决策的制定提供参考。方法选取2022年1月—2023年9月于珠海市人民医院就诊的带状疱疹患者共116例,根据患者意愿,分为A组(57例)和B组(59例)。A组患者采取基础治疗(盐酸伐昔洛韦片+甲钴胺片)+加味瓜蒌散+毫火针治疗,B组患者采取基础治疗(盐酸伐昔洛韦片+甲钴胺片)+红光照射治疗。观察2组患者PHN的发生率、肿瘤坏死因子-α(TNF-α)水平、匹茨堡睡眠质量指数(PSQI)评分。结果A组患者PHN发生率为12.3%(7/57),低于B组的33.9%(20/59),差异有统计学意义(χ^(2)=7.587,P=0.006)。治疗前,2组患者TNF-α水平比较,差异无统计学意义(P>0.05);治疗后,2组患者TNF-α水平均低于治疗前,且A组患者低于B组,差异均有统计学意义(P<0.05)。治疗前,2组患者PSQI评分比较,差异无统计学意义(P>0.05);皮疹愈合1个月后,2组患者PSQI评分均降低,且A组低于B组,差异均有统计学意义(P<0.05)。结论加味瓜蒌散联合毫火针治疗可以降低带状疱疹患者血清TNF-α水平,改善患者睡眠质量,降低PHN发生率,值得临床推广。
Objective To explore the effect of modified fructus trichosanthis powder combined with the milli-fire needle in preventing postherpetic neuralgia(PHN)in patients with herpes zoster,aiming to provide a reference for clinical decisionmaking.Methods A total of 116 patients with herpes zoster who visited Zhuhai People's Hospital from January 2022 to September 2023 were selected and allocated to Group A(57 cases)and Group B(59 cases)following their willingness.Patients in Group A received basic treatment(valacyclovir hydrochloride tablets+mecobalamin tablets)combined with modified fructus trichosanthis powder and milli-fire needle treatment,while patients in Group B received basic treatment(valacyclovir hydrochloride tablets+mecobalamin tablets)combined with red light irradiation treatment.The incidence of PHN,levels of tumor necrosis factor-α(TNF-α),and Pittsburgh Sleep Quality Index(PSQI)scores were observed in the two groups.Results The incidence of PHN in Group A was significantly lower than that in Group B[12.3%(7/57)vs.33.9%(20/59);χ^(2)=7.587,P=0.006].No significant difference was noted in TNF-α levels between the two groups before treatment(P>0.05).The TNF-α levels in both groups were decreased after treatment,and the levels in Group A were significantly lower than those in Group B(P<0.05).No statistically significant difference was found in PSQI scores between the two groups before treatment(P>0.05).One month after the rash healed,the PSQI scores in both groups significantly decreased,and the scores in Group A were significantly lower than those in Group B(P<0.05).Conclusion The treatment with modified fructus trichosanthis powder combined with the milli-fire needle can reduce the serum TNF-αlevels in patients with herpes zoster,promote sleep quality,and lower the incidence of PHN,which is worth promoting in clinical practice.
