预防脑卒中患者足下垂的循证康复方案构建与实践

Construction and practice of evidence-based rehabilitation program to prevent foot drop in stroke patients

摘要 目的:构建能够预防脑卒中患者足下垂的循证康复方案,为临床护理中防控足下垂的发生提供理论依据。方法:利用循证医学理论为基础,参照专家意见筛选适合脑卒中患者足下垂的预防的循证实践方案。采用便利抽样法选取2018年5月至2021年5月就诊西安市中心医院的120例首发脑卒中住院患者,对2018年5月至2019年11月就诊西安市中心医院的60例患者进行基线审查,通过实施前期构建的脑卒中患者足下垂循证康复方案来促进实践变革,对2019年12月至2021年5月就诊的60例患者进行再审查,追踪随访12个月,比较循证证据应用先后患者的足下垂发生情况、身体平衡功能、步行能力。结果:证据应用后,观察组患者伸肌痉挛、足下垂总发生率分别为5.00%(3/60)、10.00%(6/60)均低于对照组(均P<0.05);两组患者护理后Berg平衡量表、Holden步行功能分级均高于护理前,但观察组患者护理后Berg平衡量表、Holden步行功能分级更高(P<0.05)。结论:基于循证医学的最佳证据能够为脑卒中患者足下垂预防提供管理策略,有助于降低脑卒中患者的足下垂发生风险,改善平衡功能及步行能力。 Objective To construct an evidence-based rehabilitation program that can prevent foot drop in stroke patients,and to provide a theoretical basis for the prevention and control of foot drop in clinical care.Methods Based on the theory of evidence-based medicine,evidence-based practice programs for the prevention of foot drop in stroke patients were screened with reference to expert opinions.Convenience sampling method was used to select 120 patients with first stroke who were admitted to Xi'an Central Hospital from May 2018 to May 2021,and a baseline review was conducted on 60 patients who were admitted to Xi'an Central Hospital from May 2018 to November 2019,and the evidence-based rehabilitation program for stroke patients with foot drop was implemented to promote practical changes.A total of 60 patients treated from December 2019 to May 2021 were re-reviewed and followed up for 12 months to compare the occurrence of foot drop,body balance function and walking ability of the patients after the application of evidencebased evidence.Results After the application of evidence-based evidence,the total incidence of extensor spasm and foot drop in the observation group were 5.00%(3/60)and 10.00%(6/60),respectively,which were lower than those in the control group(P<0.05).The Berg balance scale and Holden walking function grade were higher in both groups than before care,but the Berg balance scale and Holden walking function grade were higher in the observation group(P<0.05).Conclusions The best evidence based on evidence-based medicine can provide a management strategy for foot drop prevention in stroke patients,and help to reduce the risk of foot drop occurrence,and improve the balance function and walking ability.
