摘要
目的 :研究正常健康人腕管内正中神经的超声回声表现。方法 :对 118例正常健康人腕管行高频超声检查 ,观察正中神经的形态、大小、解剖关系及回声表现 ;且在桡骨远端、豌豆骨及钩状骨切面测量正中神经的前后、左右径及截面积 ,并计算正中神经的扁平率和肿胀率。结果 :高频超声能纵、横向显示正中神经。横断面上正中神经呈圆形或类圆形低回声区 ,周围包绕以稍强回声 ;纵断面上 ,正中神经呈条带状弱回声。本组正中神经的前后、左右径及截面积分别为 :桡骨远端平面 4 2 5± 0 5 0mm、 1 92± 0 2 0mm、 6 0 4± 1 0 0mm2 ;豌豆骨平面5 0 8± 0 4 6mm、 1 88± 0 2 0mm、 7 19± 1 2 3mm2 ;钩骨平面 5 6 8± 0 6 2mm、 1 5 8± 0 2 0mm、 6 3± 1 17mm2 。正中神经平均扁平率和腕横韧带的膨隆率分别为 :桡骨远端平面 2 2 4± 0 4 1;豌豆骨平面 2 73± 0 4 5、 1 91±0 77mm ;钩骨平面 3 0 5± 0 4 7、 1 82± 0 5 9mm。正中神经平均肿胀率 :16 2 0± 0 17。结论 :高频超声能清楚显示腕管内正中神经的形态、大小及其关系 ,并能测量正中神经截面积等指标 。
Objective:To study the ultrasonic appearances and measure the dimensions of the median nerve in the normal carpal tunnel by using high-resolution ultrasonography,in order to establish the normal reference ranges.Materials and Methods:High-frequency ultrasonography examinations were performed on 236 normal carpal tunnels of 118 cases,focusing on the ultrasonic appearance,shape,size and anatomic relationship of the median nerve,Following measurements were done:(1)the anteroposterior and mediolateral diameters and the cross-sectional areas of median nerve were measured at three points:the distal radius,the pisiform bone,and the hook of the hamate bone,and(2)calculation of the flattening rate and swelling rate of the median nerve.Result:High-frequency ultrasound clearly depicted the median nerve and the flexor tendons on both transverse and sagittal planes.The median nerve was shown to be hypoechoic round or oval dots on cross sectional images,surrounded by a zone of slightly hyperechogenicity.On the other hand,the flexor tendons were slightly hyperechoic round or oval structures with a narrow hypoechogenic rim.Both appeared as a linear structures at longitudinal planes with similar echo features as at the transversal planes.The anteroposterior and mediolateral diameters and the sectional areas of the median nerve were:4 25±0 50mm?1 92±0 20mm?6 04±1 00mm 2 at the distal radius,5 08±0 46mm?1 88±0 20mm?7 19±1 23mm at the pisiform bone,and 5 68±0 62mm?1 58±0 20mm?6 63±1 17mm 2 at the hamate hook,respectively.The mean flattening rate of the median nerve and the PD of the flexor retinaculum were 2 24±0 41 at the distal radius,2 73±0 45 and 1 91±0 77mm at the pisiform bone,and 3 05±0 47 and 1 82±0 59mm at the level of the hamate hook,respectively.The swelling rate of the median nerve was 1 20±0 17.Conclusion:High-frequency ultrasonography depicted clearly the configuration,dimension and anatomical relationship of the median nerve and flexor tendons in the carpal tunnel,and enabled the accurate measurement of the median nerve.Such information would be very helpful in the diagnosis and management of patients with carpal tunnel syndrome.
