摘要
目的 :探讨拉米夫定耐药者 YMDD变异类型、变异率及与体内病毒水平的关系。方法 :荧光定量 PCR检测拉米夫定耐药者血清 HBV-DNA浓度 ,PCR产物直接测序检测 YMDD变异情况。结果 :5 2例拉米夫定耐药者 YMDD变异检测结果 ,其中有 2 4例发生 YMDD变异 ,占 46.1 5 % ,并且 YMDD(酪氨酸、蛋氨酸、天门冬氨酸、天门冬氨酸 )基序发生三种变异 ,第一种类型为 YVDD变异 ,共有 1 1例 (变异率 2 1 .1 5 % ) ,即核苷酸 A741→ G变异 ;第二种类型为 YIDD变异 ,共有 1 1例(变异率 2 1 .1 5 % ) ,即核苷酸 G743→ T变异 ,其中有 1例核苷酸 G743→ C变异也引起 YIDD变异 ;第三种类型为 YLDD变异 ,共有 2例 (变异率 3 .85 % ) ,即核苷酸 A741→ T变异。 YMDD野生型血清病毒浓度为 7.0× 1 0 6± 1 .3× 1 0 6 copies/ ml,YVDD、YIDD、YLDD变异型病毒浓度分别为 7.5×1 0 6 ± 1 .5× 1 0 6 copies/ ml、7.6× 1 0 6 ± 1 .7× 1 0 6 copies/ ml、6.9× 1 0 6 ± 1 .3× 1 0 6 copies/ ml;经 t检验 ,YMDD变异型与 YMDD野生型血清病毒浓度之间的差异无显著性意义 (P>0 .0 5 )。结论 :除已报道的拉米夫定耐药者 YMDD基序发生 YVDD和 YIDD变异外 ,还存在 YLDD变异 ;拉米夫定耐药者体内乙肝病毒水平与发生
Objective: To explore the relationship between YMDD types,variation rates and viral levels in Lamivudine resistant cases.Methods:HBV-DNA was detected by using fluorescent quantitative PCR.PCR products Sequence was detected to see the YMDD variations.Results: Out of 52 Lamivudine resistant cases,YMDD variation happened in 24 cases (46.15%) with 3 types of variations:YVDD (11 cases,21.15%),nucleic acid A741 became G;YIDD (11 cases,21.15%),nucleic acid G743 became T; YLDD(2 cases,3.85%),nucleic acid A741 became T.Viral concentration in wild YMDD was 7×10 6 copies/ml,No difference was observed between them( P >0.05). Conclusion:At least 3 types of variation were observed in Lamivudine resistant cases and no relationship was observed between HBV-DNA level,YMDD types and variation rates.
