摘要
目的 :探讨早期经空肠营养对急性胰腺炎肠源性激素分泌及效应的影响。 方法 :15只杂种犬 ,在急性胰腺炎模型制成后 2 4h分为肠外营养 (PN)组和早期经空肠营养 (EIN)组。分别在营养支持前及支持后 30、6 0、12 0和 180min时相点测定外周血中缩胆囊素 (CCK)、促胰液素 (SEC)、胃泌素等含量及 180min内胆汁、十二指肠液、胰液量及其淀粉酶、脂肪酶、HCO3 -、Cl-、Na+ 、K+ 的含量。 结果 :EIN组CCK含量在输注营养液 6 0min及以后各时相点均高于PN组 (P <0 .0 5 ) ;两组SEC在营养液输注后均高于输注前 (P <0 .0 5 ) ,6 0min时相点高于PN组 ,随后又下降 ,两组间差异不显著 (P >0 .0 5 ) ;EIN组胃泌素指标有逐渐上升趋势 ,在 12 0、180min时相点高于PN组(P <0 .0 5 ) ;观察时间内两组间胆汁、十二指肠液、胰液等含量及其淀粉酶、脂肪酶含量和电解质HCO3 -、Cl-、Na+ 、K+ 含量差异均不显著。 结论 :EIN在某种程度上能刺激肠源性激素的分泌 ,但并没有过多地刺激消化液分泌。
Objectives:This study was to evaluate the effect of early intrajejunal nutrition(EIN) on secretion of entero hormone and its efficiency in dogs with acute pancreatitis. Methods:Fifteen dogs with acute pancreatitis induced by retrograde injection of 5% sodium taurocholate into the pancreatic duct were randomly invided into parenteral nutrition(PN) group( n =7) and EIN group( n =8).The levels of cholecystokinin(CCK), secretin(SEC) and gastrin in the peripheral plasma were measured by ELISA and RIA,and the quantitative analysis of biliary juice, duodenal juice, pancreatic juice, amylase, pancreatolipase,HCO 3 -,Cl -,Na + and K + by auto biochemical analyzer at the beginning,or 30,60,120 and 180 min after PN or EIN were performed. Results:The plasma levels of CCK and gastrin in EIN group were higher than those in PN group at 120 and 180 min( P <0.05).The changes of biliary juice,duodenal juice,pancreatic juice,amylase,pancreatolipase,HCO 3 -,Cl -,Na + and K + bewteen two groups did not reach statistically significant difference( P >0.05). Conclusions:EIN may stimulate the secretion of some entero hormons,but it do not increase the secretion of pancreatic juice and pancreatic enzymes.
