摘要
目的 观察自体多股半腱肌半膜肌 (STG)重建膝前交叉韧带术后早期快速进展康复治疗的安全性和对患者膝功能水平的影响。方法 单侧自体多股半腱肌半膜肌膝前交叉韧带重建术 48例 ,其中康复组 31例术后接受快速进展康复方法 ,对照组 1 7例。比较两组术后 6月的关节活动度、肌力、Lysholm评分及关节稳定性检查。结果 术后 6月康复组与对照组伸展受限分别是 2 .63°±2 .50°和 4 .1 5°± 2 .65°,屈曲度减少分别是 4.1 5°± 2 .95°和 5.0 1°± 2 .41° ,P <0 .0 5。Cybex肌力测试康复组与对照组伸肌 (患肢 /健肢 ) 71 .57± 6 .82 %vs62 .62± 7.54% ,屈肌 (患肢 /健肢 ) 90 .0 1± 8.75%vs 75.2 9± 9.38% ,P <0 .0 0 1。康复组Lysholm评分90 .1± 5 .1 5 ,对照组 82 .4± 7.56 ,P <0 .0 0 1。关节稳定性检查 ,术后 6月两组患膝Lanchman和Pivotshift检查均阴性。结论 我们进行的术后快速进展早期康复方法能促进自体多股STG重建膝交叉韧带患者功能水平的恢复 。
Objective To evaluate the effects of early rehabilitation on the functional recovery in patients after anterior cruciate ligament reconstruction with STG. Methods Fourty eight patients were divided into two groups. Patients in the group I( n =31) underwent early rehabilitation for 4～6 months post operation. The patients in group II ( n =17) who received traditional treatment served as control. The knee range of motion, Lysholm score and knee laxity test were evaluated for all these patients. The muscle strength of knee during extension and flexion was tested by using Cybex. Results The deficit during either extension or flexion was significantly smaller in group I than that in the control group (2.63°±2.50° vs 4.15°±2.65°,4.15°±2.95° vs 5.01°±2.41°,respectively). Patients in group I achieved better muscle strength, and the Lysholm score was higher as compared with that in patients in group I. Six months after the operation, all patients had negative Lanchman and Pivot shift test. Conclusion The results suggest that early rehabilitation may improve the patient's knee function after reconstruction of the anterior cruciate ligament.
