摘要
目的 介绍以拇指尺背侧动脉为蒂逆行皮瓣的手术方法和适应证。方法 对 5例拇指远端软组织缺损并伴骨外露者 ,采用以拇指尺背侧动脉为蒂的皮瓣逆行转移修复。该皮瓣的设计以拇指尺背侧动脉体表投影线为轴心线 ,距甲沟 2 .3cm处纵轴线向背侧动脉穿支为旋转点 ,皮瓣面积可达 3.0cm× 2 .5cm。结果 术后 5例皮瓣全部成活。术后随访 2～ 6个月 ,拇指外形满意 ,指腹二点分辨觉为6～ 10mm。指间关节活动范围从 0°～ 70°左右。
Objective To introduce the surgical procedure and indication for the dorsoulnar arterial retrograde flap of the thumb. Methods Five cases of soft tissue defect with exposed bone at the distal thumb were involved in the study. The flap made the surface projection of the dorsoulnar artery of the thumb as axial line, and the perforating branch of dorsal artery 2.3 cm from nail groove at the vertical line as rotation point. The areas of flap were 3.0 cm × 2.5 cm.Results All 5 flaps survived.At 2 to 6 months follow-up postoperatively, appearance of the thumb was satisfactory, with 6 to 10 mm in 2-PD and 0°～70° in range of motion at IP joint of the thumb. Conclusions The optimal indication for the procedure is skin and soft tissue defects combined with exposed bone at the distal thumb.
