摘要
目的探讨乏特壶腹周围癌围手术期处理与术后并发症关系。方法对1989-01～2001-07共67例壶腹周围癌患者进行回顾性分析,并对术后并发症发生原因和治疗方法进行总结。结果①并发症的发生与梗阻性黄疸患者的高胆红素血症、内毒素血症及肝功能受损有密切关系;②术前减黄手术与并发症发生率无关(P>0.05);③外引流术作为姑息手术并不理想,内引流术才是减黄的理想方法。结论加强围手术期处理可提高壶腹周围癌患者的手术耐受性,减少术后并发症。
Objective To explore the relationship of perioperative management of peripheral carcinoma of Vater's ampulla and postoperative complication.Methods From Jan.,1989to July,2001,a retrospective analysis was made on67patients with periopheral carcinoma of ampulla.The causes of postoperative complication and the methods of management were summarized.Results The occurrence of complications was closely related with hyperbilirubinemia,endotoxemia and liver dysfunction,but not with decompression of bile duct(P>0.05);Internal drainage was better than external drainage for palliative therapy.Conclusions To enhance the perioperative management can improve the supportable ability of patients with peripheral carcinoma of ampulla and can decrease the post operative complication
