摘要
目的分析CT增强扫描检查中对比剂外渗的影响因素,为临床预防对比剂外渗提供参考依据。方法回顾性分析郑州市第六人民医院2022年9月至2023年7月收治的104例需进行CT增强扫描检查的受检者临床资料,根据其是否发生对比剂外渗情况,将受试者分为未发生组(85例)、发生组(19例)。分析受试者临床资料,对可疑的影响因素单因素方差分析,筛选出有差异的因素再进行二元Logistic回归分析,总结CT增强扫描检查中对比剂外渗的影响因素。结果对比剂外渗发生组年龄≥60岁、有高血压、糖尿病、恶性肿瘤、碘对比剂浓度>370 mg/ml、注射速率≥3 ml/s、注射人员资历≤2年、有化疗史受试者占比与未发生组占比升高(P<0.05)。二元Logistic分析显示,年龄≥60岁、有高血压、糖尿病、恶性肿瘤、碘对比剂浓度>370 mg/ml、注射速率≥3 ml/s、注射人员资历≤2年、有化疗史是CT增强扫描检查中对比剂外渗的影响因素(OR值=3.165、4.646、7.471、6.398、7.614、5.073、6.392、2.779)(P<0.05)。结论年龄≥60岁、有高血压、糖尿病、恶性肿瘤、碘对比剂浓度>370 mg/mL、注射速率≥3 ml/s、注射人员资历≤2年、有化疗史是CT增强扫描检查中对比剂外渗的影响因素,临床可对有以上临床特征的受试者进行密切关注,并积极开展实施针对性防护措施,以降低CT增强扫描检查中对比剂外渗的发生风险。
Objective To analyze the influencing factors of contrast agent extravasation in contrast-enhanced CT scanning and provide a reference for clinical prevention of contrast agent extravasation.Methods A retrospective study was conducted on the clinical data of 104 patients admitted to Zhengzhou Sixth People′s Hospital from September 2022 to July 2023 who required enhanced CT scanning.Their clinical data were collected,and they were divided into a non-occurrence group(85 cases)and an occurrence group(19 cases)based on whether they had extravasation of contrast agents.By analyzing the clinical data of the subjects and screening suspicious influencing fact-ors,one-way ANOVA should be used to screen out the factors with differences,and then binary Logistic regression analysis should be performed to find out the influencing factors of contrast agent extravasation in CT-enhanced scanning.Results The proportion of subjects with age≥60 years,hypertension,diabetes,malignant tumor,iodine contrast concentration>370 mg/ml,injection rate≥3 ml/s,qualification of injection personnel≤2 years and history of chemotherapy increased compared with the control group(P<0.05).Logistic analysis showed that age≥60 years,hypertension,diabetes,malignant tumor,iodine contrast concentration>370 mg/ml,injection rate≥3 ml/s,qualification of injection personnel≤2 years and chemotherapy history were the influencing factors of contrast extravasation in CT enhanced scanning(OR=3.165,4.646,7.471,6.398,7.614,5.073,6.392,2.779)(P<0.05).Conclusion Age≥60 years,hypertension,diabetes,malignant tumor,iodine contrast concentration>370 mg/ml,injection rate≥3 ml/s,qualification of injection personnel≤2 years and history of chemotherapy are the influencing factors of contrast agent extravasation in CT enhanced scanning.Clinicians can pay close attention to subjects with the above clinical characteristics,and actively implement targeted protective measures,to reduce the risk of contrast agent leak-age during enhanced CT scanning.
