骨水泥联合克氏针自制外固定架治疗中节指骨Pilon骨折 被引量:1

Application of self-made external fixator using bone cement combined with Kirschner wire for the treatment of middle phalange Pilon fractures

摘要 目的 探讨骨水泥联合克氏针自制外固定架治疗中节指骨Pilon骨折的疗效,以期取得对该类骨折更好的治疗方法.方法 自2021年1月至2023年1月,我们共收治14例中节指骨Pilon骨折患者,采用骨水泥联合克氏针自制外固定架手术治疗,术后2、4、6及12周摄X线片,并进行手指总主动活动度(total active motion,TAM)、近指间关节(proximal interphalangeal joint,PIP)活动度评分系统评估,测量记录视觉模拟评分(visual analogue scale,VAS).结果 术后所有患者均获得随访,时间为6~12个月,平均8.8个月,术后6周骨折全部愈合,术后12周TAM评分为(206.4±4.9)°,PIP活动度为(70.71±3.5)°,VAS平均为(4.21±0.34)分.结论 骨水泥联合克氏针自制外固定架治疗中节指骨Pilon骨折技术简单,可以提供牵引下坚强固定,效果可靠. Objective To explore the clinical efficacy of self-made external fixator using bone cement combined with Kirschner wire for the treatment of middle phalange Pilon fractures,in order to obtain a better treatment method for this type of fractures.Methods From January 2021 to January 2023,a total of 14 patients with middle phalange Pilon fractures were treated by self-made external fixator using bone cement combined with Kirschner wire.X-ray films were taken at 2,4,6,and 12 weeks postoperatively,and total active motion(TAM)of the fingers and the proximal interphalangeal joint(PIP)activity scoring system were evaluated.The visual analogue scale(VAS)was measured and recorded.Results All patients were follow-up from 6 to 12 months with an average of 8.8 months after surgery,and the fractures healed at 6 weeks postoperatively.The TAM score at 12 weeks postoperatively was(206.4±4.9)°,PIP activity was(70.71±3.5)°,and VAS average was(4.21±0.34)points.ConclusionThe technique of self-made external fixator using bone cement combined with Kirschner wire to treat middle phalange Pilon fractures is simple,which can provide strong fixation under traction,and the effect is reliable.
