摘要
叶天士是清朝著名医家,其在《临证指南医案》中论治泄泻的内容丰富,其中不乏阴虚泄泻的医案。文章主要通过对《临证指南医案》泄泻一门中关于阴虚泄泻的医案进行整理分析,从而对叶氏论治阴虚泄泻的思想进行归纳总结。叶氏认为,阴虚泄泻的病机主要包括阴弱不能包涵(水谷精微)及厥阴阳明不和两方面,其中又以后者为叶氏学术特色。为此,叶氏立苦味坚阴、淡渗利湿,酸甘化阴、肝胃同治,酸苦泄热、抑木扶土等法,对后世颇具启发意义。
YE Tianshi is a famous doctor in the Qing Dynasty.He has rich contents on the treatment of diarrhea in his“Clinical Guide to Medical Cases”,among which there are many medical cases about Yin deficiency diarrhea.In this paper,we sort out and analyze the medical cases of Yin deficiency diarrhea in“Clinical Guide to Medical Cases”,so as to summarize Ye's thought of treating Yin deficiency diarrhea.YE believes that the pathogenesis of Yin deficiency diarrhea mainly includes two aspects:Yin weakness can not contain(the essence of water and grain)and Jue Yin and Yang Ming discordance,the latter of which is Ye's academic feature.Therefore,YE's methods of establishing bitter taste firming Yin,light seepage to diuresis,acid and sweet producing Yin,treating liver and stomach together,acid and bitter to relieve heat,suppressing wood and supporting soil are of great enlightening significance to later generations.
