摘要
蛛网膜下腔出血(subarachnoid hemorrhage,SAH)是一种危重的出血性神经重症,其致死率高,常伴有多种并发症,如脑血管痉挛(cerebral vasospasm,CVS)、迟发性脑梗死和脑积水等。SAH属出血性中风的一种,中医学主张早期运用活血化瘀治疗。SAH的病理机制较为复杂,包括氧化应激、细胞凋亡和炎症反应等。西医药物治疗对改善SAH患者预后效果并不理想。中药川芎多用于活血行气和缓解头痛,研究证实,川芎及其活性成分具有抗炎、镇痛、舒张血管等多重药理作用。大量研究表明,川芎及其活性成分可通过多靶点、多途径防治SAH,但是缺乏系统、全面的总结与分析。该文针对川芎及其活性成分治疗SAH作用机制的相关文献进行分析归纳。分析结果表明,川芎对SAH确有治疗效果,川芎主要通过抗氧化应激反应、抑制细胞凋亡、抗炎症反应、减轻血脑屏障破坏及缓解CVS几个方面发挥出对SAH的治疗效果,其中以川芎嗪为代表的活性成分效果尤为显著。
Subarachnoid hemorrhage(SAH)is a severe hemorrhagic neurocritical condition with a high mortality rate,often accompanied by various complications such as cerebral vasospasm(CVS),delayed cerebral infarction,and hydrocephalus.SAH is considered a type of hemorrhagic stroke,and in traditional Chinese medicine(TCM),it is advocated to treat it early with therapies aimed at activating blood circulation and resolving stasis.The pathogenesis of SAH is complex,involving mechanisms such as oxidative stress,apoptosis,and inflammatory responses.Western medical pharmacological treatments have not been very effective in improving the prognosis of patients with SAH.Chuanxiong(Chuanxiong Rhizoma),a traditional Chinese medicinal herb,is commonly used for its effects in promoting Qi and blood circulation,as well as for its wind-dispelling and pain-relieving properties.Studies have confirmed that Chuanxiong(Chuanxiong Rhizoma)and its active components possess multiple pharmacological effects,including anti-inflammatory,analgesic,and vasodilatory actions.Extensive research indicates that Chuanxiong(Chuanxiong Rhizoma)and its active ingredients can prevent and treat SAH through multitarget and multi-pathway approaches;however,a systematic and comprehensive summary and analysis are still lacking.This article aims to analyze and summarize the literature on the mechanisms of action of Chuanxiong(Chuanxiong Rhizoma)and its active components in the treatment of SAH.The analysis results demonstrate that Chuanxiong(Chuanxiong Rhizoma)does indeed have therapeutic effects on SAH,primarily through its antioxidant stress response,inhibition of apoptosis,anti-inflammatory reactions,reduction of blood-brain barrier damage,and alleviation of CVS.Among these,the active component tetramethylpyrazine(TMP)stands out for its significant effects.
