合肥市某城市社区老年人血铊与代谢综合征的关联性研究

摘要 目的探讨城市社区老年人血铊与代谢综合征(metabolic syndrome,MS)之间的关联。方法2022年3—10月,在安徽省合肥市某个社区抽取≥60岁老年人进行问卷调查、体格检查及实验室检测。根据美国国家胆固醇教育计划成人治疗方案第三次报告标准诊断MS。使用电感耦合等离子体质谱仪对全血铊浓度进行测量,并将血铊浓度等分为四组。采用单因素和多因素logistic回归模型分析老年人血铊与MS的关联,采用限制性立方样条进一步分析两者的剂量-反应关系。结果共纳入1008名老年人,平均年龄为(71.87±6.46)岁。MS患病率为43.95%(443人)。血铊检出率为92.26%,浓度中位数为0.083μg/L。调整性别、年龄、文化程度、职业、婚姻状况、吸烟状况、饮酒状况、锻炼状况、饮食情况、盐摄入情况、油摄入情况、糖摄入情况、高血压、糖尿病和体质指数后,血铊浓度与MS呈显著的正相关(ORQ2 vs.Q1=1.787,95%CI:1.057~3.021;ORQ3 vs.Q1=2.120,95%CI:1.236~3.626;ORQ4 vs.Q1=2.046,95%CI:1.219~3.438)。限制性立方样条曲线显示,血铊浓度与老年人MS存在非线性剂量-反应关系:随着铊浓度的增加,MS风险先显著升高,后逐渐降低。结论铊暴露可能是老年人发生MS的独立危险因素。鉴于铊在日常生活、工业以及医疗产品中的广泛应用,其对MS发生的病因学和公共卫生意义值得关注。 Objective To explore the association between blood thallium(Tl)and metabolic syndrome(MS)among urban community-dwelling senior citizens.Methods From March to October 2022,senior citizens aged 60 years and above were selected from a community in Hefei City,Anhui Province to conduct a questionnaire survey,physical examination and laboratory tests.Diagnosis of MS was based on the standards of Adult Treatment Panel III(ATP III),USA.The concentrations of Tl in whole blood were measured by inductively coupled plasma-mass spectrometer(ICP-MS),and then divided into four groups.Univariate and multivariate logistic regression models were used to analyze the correlation between blood Tl and MS in the senior citizens,andrestricted cubic spline(RCS)was employed to further analyze the dose-response relationship between them.Results A total of 1,008 senior citizens were enrolled in this study,with the mean age of(71.87±6.46)years.The prevalence rate of MS was 43.95%(443/1,008).The detection rate of blood Tl was 92.26%,and the median concentration 0.083μg/L.After adjustment for gender,age,educational background,occupation,marital status,status of smoking,alcohol consumption,physical exercise,diet,salt intake,oil intake and sugar intake,hypertension,diabetes,and body mass index(BMI),there was a significant positive correlation between blood Tl and MS(ORQ2 vs.Q1=1.787,95%CI:1.057-3.021;ORQ3 vs.Q1=2.120,95%CI:1.236-3.626;ORQ4 vs.Q1=2.046,95%CI:1.219-3.438).The restricted cubic spline curve showed a non-linear dose-response relationship between blood Tl and MS:the risk of MS significantly increased and then gradually decreased with the increase in blood Tl concentration.Conclusion Exposure to Tl may be an independent risk factor for MS in the senior citizens.Given the widespread application of Tl in daily life,industry and medical products,its etiological and public health implications for the occurrence of MS are worthy of attention.
