摘要
目的分析1例假巴西诺卡菌感染引起脓胸的病例,探讨假巴西诺卡菌的形态特点、鉴定方法、临床表现、影像学特征及临床诊治方案,为病原菌的分离鉴定和诊断治疗提供经验。方法回顾并分析武汉市肺科医院收治的1例假巴西诺卡菌引起的脓胸患者资料及临床治疗方案,并复习相关文献。结果根据检验结果及药敏信息及时更改美罗培南为复方新诺明联合阿莫西林/克拉维酸钾治疗,该患者诊疗后症状缓解,提前出院。结论对假巴西诺卡菌引起的肺部疾病尤其是脓胸患者的及时诊断和治疗具有重要意义。
Nocardia pseudobrasiliensis(N.pseudobrasiliensis)is a new taxon constituting an emerging species of human pathogenic Nocardia.Few clinical cases of N.pseudobrasiliensis infection have been reported.Here,we discuss the morphological features,identification methods,clinical manifestations,clinical diagnosis,and treatment of N.pseudobrasiliensis,describing experience in the isolation and identification of this pathogen.The case information and diagnostic process in a patient with pyothorax caused by N.pseudobrasiliensis who received treatment in Wuhan Pulmonary Hospital was analyzed retrospectively.The relevant literature was analyzed,and experience in diagnosis and treatment is discussed.The treating physician promptly changed the treatment from meropenem to trimethoprim-sulfamethoxazole combined with amoxicillin/clavulanic acid,on the basis of test results and drug sensitivity information.The patient was discharged early with symptomatic relief after treatment.The reporting of this case is aimed at increasing clinicians'awareness of the disease,decreasing misdiagnosis and underdiagnosis,and supporting timely diagnosis and treatment of patients with lung diseases caused by N.pseudobrasiliensis,particularly pyothorax.
