摘要
支气管扩张症的发病机制复杂,气道炎症、黏液高分泌及气道重塑被认为是支气管扩张症重要的气道病理生理特征,常常互相影响,形成恶性循环,促使支气管扩张症的发生和发展。文章从气道炎症、黏液高分泌和重塑角度对支气管扩张症的病理生理机制作一综述,为临床治疗提供理论依据,以期对关键作用靶点的干预,能有效应对该病的治疗。目前部分临床研究提示有些中西药对支气管扩张症气道炎症、黏液高分泌及气道重塑有一定的干预作用,但多数研究样本量小,缺乏充分可靠的循证医学依据,药物的疗效及安全性尚需多中心大样本随机双盲对照试验进一步证实。
The pathogenesis of bronchiectasis is still not well defined.Airway inflammation,mucus hypersecretion and airway remodeling are considered as important pathophysiological features of bronchiectasis.Persistent airway inflammation,airway mucus hypersecretion and airway remodeling often interact to form a vicious circle,promoting the occurrence and development of bronchiectasis.This article reviewed the pathophysiological mechanism of bronchiectasis from the perspective of airway inflammation,mucus hypersecretion and remodeling,so as to provide theoretical basis for clinical treatment,with a view to intervening key target points and effectively coping with the treatment of bronchiectasis.At present,some clinical studies suggest that some traditional Chinese and western drugs have certain intervention effects on airway inflammation,mucus hypersecretion and airway remodeling in bronchiectasis,but most of the studies have small sample size,and still lack sufficient and reliable evidence-based medical basis.The efficacy and safety of the drugs need to be further confirmed by multi-center,large sample and randomized double-blind controlled experiment.
