摘要
神经性耳鸣是一种耳鼻喉科常见的临床症状,由于其发病机制尚不清楚,目前仍无理想的治疗方法,患者因耳鸣引发睡眠障碍、头晕、便秘等多种不适症状。发病机制尚不明了,普遍认为与噪声、振动、温度、辐射等因素密切相关。耳鸣属中医学“脑鸣”“蝉鸣”等范畴。中医学认为,肾开窍于耳,肝肾同源,耳与肝肾关系甚为密切,肾精亏虚,髓海不足,基本病机为耳窍失于充养而致耳鸣。耳鸣可分为虚实两类,实证有外邪侵犯型、肝火上扰型、痰浊上壅型等,虚证有肝肾不足型、脾胃虚弱型等。现代西医学对神经性耳鸣的治疗暂无特效药物,而中医通过辨证分型论治选用不同类型中药汤剂、针刺、揿针、运动灸、推拿、小针刀等多种方式治疗神经性耳鸣,安全性高、见效快、效果持久。
Neurogenic tinnitus(NT)is a common clinical symptom in otolaryngology.Due to its unclear pathogenesis,there is currently no ideal treatment method for NT.NT Patients may experience various discomfort symptoms,such as sleep disorders,dizziness,and constipation.The pathogenesis is still unclear,and it is generally believed that the onset of NT is closely related to noise,vibration,temperature,and radiation.Tinnitus belongs to the categories of"brain ringing"and"cicada ringing"in traditional Chinese medicine(TCM).The TCM theory believed that the kidney opens at the ears,and the liver and kidney have a common source.The relationship between the ear and the liver and kidney is very close.The kidney-jing deficiency and insufficiency of marrow-sea due to insufficient nutrition of external acoustic meatus result in tinnitus.Tinnitus can be divided into two categories:deficiency and excess.The excess syndrome of tinnitus includes the syndrome types of external invasion,disturbing upward of liverfire and phlegm stagnation.The deficiency syndrome includes the syndrome types of deficiency of liver and kidney,and weakness of the spleen and the stomach.Effective western medicine drugs for NT are scant.Diagnosis and treatment based on syndrome differentiation and classification of TCM,NT can be treated with multiple TCM methods,such as decoctions,acupuncture,press needling,sports moxibustion,massage and needle knife,showing high safety,fast effect and long-lasting outcome.
