酸蚀、喷砂及Er:YAG激光处理恒牙釉质表面的临床研究 被引量:2

摘要 目的探讨比较酸蚀、喷砂及Er:YAG激光处理恒牙釉质表面,对牙釉质表面形态、树脂剪切粘结强度及比较各组激光荧光诊断仪读数(LF)值的影响。方法选取牙体健康无龋坏的50颗成人离体前磨牙或磨牙,按近远中方向将离体牙切割成颊/舌两部分,最终选取60个样件,按不同的表面处理方法分为空白对照组、喷砂组、酸蚀组、Er:YAG激光组和酸蚀+Er:YAG激光联合组,每组12个样件。每组2个样件扫描电镜观察,5个样件进行剪切强度测试,其他样件进行LF读数测定。结果酸蚀组、激光组、联合组的剪切强度值均分别高于空白对照组(P<0.05);喷砂组剪切强度与空白对照组差异无统计学意义(P>0.05);联合组、Er:YAG激光组的剪切强度值均分别高于酸蚀组,差异有统计学意义(P<0.05),联合组和Er:YAG激光组比较,差异无统计学意义(P>0.05);酸蚀组、激光组及联合组LF值均低于空白对照组,差异有统计学意义(P<0.05)。结论Er:YAG激光和酸蚀处理牙釉质表面后,均能增强表面粗糙度,增加牙釉质剪切粘结强度,降低LF值,激光处理效果佳于酸蚀处理,两者联合使用效果更佳。 Objective To compare the effects of acid etching,sandblasting and Er:YAG laser treatment on the surface morphology of permanent enamel,resin shear bonding strength and the reading value of laser fluorescence(LF)diagnostic instrument in each group.Methods A total of 50 healthy and caries free adult extracted premolars or molars were selected,and the extracted teeth were cut into buccal/lingual sections in the mesial and distal directions.Finally,60 samples were selected and divided into a blank control group,sandblasting group,acid etching group,Er:YAG laser group,and acid etching+Er:YAG laser combination group according to different surface treatment methods,with 12 samples in each group.Two samples in each group were observed under scanning electron microscopy,five samples were subjected to shear strength testing,and the remaining samples were measured for LF readings.Results The shear strength values of the acid etching group,laser group,and combination group were all higher than those of the blank control group(P<0.05).There was no significant difference in shear strength between the sandblasting group and the blank control group(P>0.05).The shear strength values of the combination group and Er:YAG laser group were higher than those of the acid etching group,with statistical significance(P<0.05).However,there was no statistically significant difference between the combination group and Er:YAG laser group(P>0.05).The LF values of the acid etching group,laser group,and combination group were all lower than those of the blank control group,with statistical significance(P<0.05).Conclusion Both Er:YAG laser and acid etching treatment can enhance surface roughness,increase enamel shear bonding strength,and reduce LF value.Laser treatment has a better effect than acid etching treatment,and the combination of the two has a better effect.
