摘要
目的:探讨腔隙性脑梗死(LI)患者血清淀粉样蛋白A(SAA)、超氧化物歧化酶(SOD)的表达及其临床意义.方法:选取2022年2月至2023年2月本院诊治的80例LI患者为研究对象.全部患者于入院时、入院后3d、入院后7d、出院时检测患者血清SAA、SOD水平,治疗结束后随访90d,采用改良Rankin量表(mRS)评估预后.并根据预后情况将这些患者分为预后不良组(n=20)和预后良好组(n=60).比较两组患者入院时血清SAA、SOD水平,并分析LI患者预后与入院时血清SAA、SOD的关系.结果:入院后各时点,LI患者血清SAA水平降低,血清SOD水平升高,各时点间比较差异均有统计学意义(P<0.05).经随访90d,80例LI患者,预后不良占25.00%(20/80);预后良好者占75.00%(60/80).预后不良组入院时血清SAA高于预后良好组,血清SOD低于预后良好组(P<0.05).构建Logistic回归分析模型,结果显示,LI患者预后与入院时血清SAA、SOD水平异常有关,入院时血清SAA过表达、SOD低表达可能是预后不良的影响因素(P<0.05).结论:LI患者存在血清SAA过表达、血清SOD低表达的情况,随着治疗有一定改善,但SAA的异常过表达、SOD异常低表达可能提示患者预后不良风险,应引起重视.
Objective:To explore the expression and clinical significance of serum amyloid A(SAA)and superoxide dismutase(SOD)in patients with lacunar infarction(LI).Methods:A total of 80 LI patients treated in our hospital from February 2022 to February 2023 were selected as the study subjects.All the patients were measured for their serum levels of SAA and SOD at admission,3 days after admission,7 days after admission,and discharge.After treatment,the patients were followed up for 90 days.At the end of the follow-up,the modified Rankin scale(mRS)was used to evaluate the patient's prognosis,and the scores were recorded.The patients were divided into poor prognosis group(n=20)and good prognosis group(n=60).The serum SAA and SOD levels at admission were compared between the two groups,and the relationship between the prognosis of LI patients and serum SAA and SOD at admission was analyzed.Results:At various time points after admission,the serum SAA level of LI patients decreased and the serum SOD level increased.The differences between different time points were statistical significant(P<0.05).After 90 days of follow-up,among 80 LI patients,the poor prognosis accounted for 25.00%(20/80),and the patients with good prognosis accounted for 75%(60/80).The patients with poor prognosis had higher serum SAA and lower serum SOD at admission compared to those with good prognosis,with a statistical significant difference(P<0.05).Logistic regression analysis model was constructed,and the results showed that the prognosis of LI patients was related to abnormal levels of serum SAA and SOD at admission.Over expression of serum SAA and low expression of SOD at admission may be factors affecting poor prognosis(P<0.05).Conclusion:LI patients have over expression of serum SAA and low expression of serum SOD,which may improve with treatment.However,abnormal over expression of SAA and abnormal low expression of SOD may indicate a risk of poor prognosis in patients and should be taken seriously.
