摘要
目的 拟分析基于最佳证据总结的快速反应团队(RRT)模式在急性缺血性脑卒中(AIS)患者救治中的应用效果。方法 选取2021—2023年我院收治的86例AIS患者作为研究对象,其中2021年1月至2022年6月收治的41例纳入对照组,2022年7月至2023年12月收治的45例纳入RRT组。对照组采取常规救治模式,RRT组在对照组基础上采取基于最佳证据总结的RRT模式。比较两组患者主要耗时指标、溶栓前后美国国立卫生研究院卒中量表(NIHSS)评分、溶栓效率及不良事件发生率。结果 RRT组入院至实验室结果获取时间、入院至CT报告获取时间及入院至给予静脉溶栓时间(DNT)均短于对照组(P均<0.05);两组入院至接诊时间比较无统计学差异(P>0.05)。两组治疗后NIHSS评分随时间呈下降趋势,且RRT组各时点NIHSS评分均低于对照组(P均<0.05)。RRT组DNT达标率、血管再通率显著高于对照组(P均<0.05);两组溶栓有效率比较无统计学差异(P>0.05)。两组不良事件发生率比较无统计学差异(P>0.05)。结论 将基于最佳证据总结的RRT模式应用于AIS患者救治中,可有效缩短救治时间,优化急救流程,改善患者功能,提高DNT达标率和血管再通率,安全可行。
Objective To analyze the application effect of the rapid response team(RRT) model based on the best evidence summary in the treatment of patients with acute ischemic stroke(AIS).Methods A total of 86 patients with AIS admitted to the hospital from 2021 to 2023 were selected as the study subjects.Among them,41 patients who were admitted from January 2021 to June 2022 in control group received conventional treatment,and the other 45 patients who were admitted from July 2022 to December 2023 received treatment with RRT model.The time consumption indicators,Stroke Scale(NIHSS) scores before and after thrombolysis,thrombolysis efficiency and adverse events were compared between the two groups.Results The time from admission to obtaining laboratory results,time from admission to obtaining CT report,and door-to-needle time(DNT) in the RRT group were shorter than those in control group(P<0.05).After treatment,the NIHSS scores of both groups showed a decreasing trend over time,and the RRT group had lower NIHSS scores than the control group at all time points(P<0.05).The standard-reaching rate of DNT and vascular recanalization rate in RRT group were significantly higher than those in control group(P<0.05).Conclusion Applying the RRT model based on the best evidence summary in the treatment of patients with AIS can effectively shorten the treatment time,optimize first-aid process,improve the patients' function,and increase the standard-reaching rate of DNT as well as vascular recanalization rate.
