摘要
目的:分析不同胎龄新生儿出生初期部分凝血功能检测指标的危急值。方法:回顾性选取2021年1月-2023年12月于单县东大医院新生儿科出生的200例新生儿作为研究对象,根据胎龄分为早产组(胎龄<37周)和足月组,各100例。两组新生儿均接受凝血功能检测,建立新生儿凝血四项指标的危急值参考范围。结果:足月组活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、凝血酶时间(TT)均短于早产组,纤维蛋白原(FIB)水平高于早产组,差异有统计学意义(P<0.001)。足月组APTT危急值参考范围为42.02~77.62 s,PT危急值参考范围为12.01~17.42 s,TT危急值参考范围为14.53~19.03 s,FIB危急值参考范围为下限1.32~2.44 g/L;早产组APTT危急值参考范围为49.14~92.44 s,PT危急值参考范围为12.54~18.94 s,TT危急值参考范围为15.41~23.51 s,FIB危急值参考范围为1.11~3.00 g/L;结论:新生儿出生初期需重视进行凝血功能检测,并积极建立指标危急值以及时进行病情预测。
Objective:To analyze the critical values of partial coagulation function in newborns of different gestational ages.Methods:A total of two hundred newborns born in the Department of Laboratory,Shandong Shanxian County Dongda Hospital from January 2021 to December 2023 were retrospectively selected as the study objects,and divided into preterm group(gestational age<37 weeks)and term group(100 cases each)according to gestational age.The two groups of newborns were tested for coagulation function,and the reference range of the four critical values of neonatal coagulation was established.Results:The activated partial thromboplastin time(APTT),prothrombin time(PT)and thrombin time(TT)in the term group were shorter than those in the preterm group,the level of fibrinogen(FIB)was higher than that of preterm group,and the difference was statistically significant(P<0.001);in the term group,the reference range of APTT critical value was 42.02-77.62 s,the reference range of PT critical value was 12.01-17.42 s,the reference range of TT critical value was 14.53-19.03 s,and the reference range of FIB critical value was the lower limit 1.32-2.44 g/L;in the preterm group,the reference range of APTT critical value was 49.14-92.44s,the reference range of PT critical value was 12.54-18.94 s,the reference range of TT critical value was 15.41-23.51s,and the reference range of FIB critical value was 1.11-3.00 g/L.Conclusion:The newborn should pay attention to the coagulation function test at the early stage of birth,and actively establish the critical value of indicators for timely disease prediction.
