摘要
目的观察靳三针联合益智填髓推拿法对儿童智力障碍(ID)肝肾不足证的治疗效果。方法选取2019年5月至2021年5月长葛市第二人民医院中医内科和儿科收治的ID肝肾不足证患儿64例,按照随机数字表法分为治疗组和对照组,各32例。对照组给予神经康复综合治疗,治疗组实施靳三针联合益智填髓推拿法治疗,2组均以1个月为1个疗程,治疗3个疗程后,统计2组的临床疗效,智力水平、运动能力、日常生活能力以及血清神经元特异性烯醇化酶(NSE)、5-羟色胺(5-HT)、多巴胺(DA)与去甲肾上腺素(SE)水平。结果治疗组的总有效率为93.75%(30/32),对照组为75.00%(27/32),2组比较,差异有统计学意义(P<0.05)。治疗前2组智力发育水平比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组智力发育水平均明显提高,与同组治疗前比较,差异有统计学意义(P<0.05),且治疗组提高更显著(P<0.05)。治疗前2组粗大运动功能测试量表(GMFM)和日常生活活动能力量表(ADL)评分比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组GMFM和ADL评分均显著升高,与同组治疗前比较,差异有统计学意义(P<0.05),且治疗组升高更显著(P<0.05)。治疗前2组血清NSE、5-HT、DA、NE水平比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组血清NSE水平显著下降,5-HT、DA、NE水平显著升高,与同组治疗前比较,差异有统计学意义(P<0.05),且治疗组下降或升高更显著(P<0.05)。结论靳三针联合益智填髓推拿法治疗儿童ID肝肾不足证临床疗效显著,不仅能显著提高ID患儿的智力水平、运动能力、日常生活能力,还可调节血清NSE及单胺类神经递质水平,值得临床推广应用。
Objective To observe the therapeutic effect of Jin's three needles combined with Yizhi Tiansui tuina therapy in the treatment of childhood intellectual disability(ID)with deficiency of liver and kidney.Methods A total of 64 children ID with deficiency of liver and kidney admitted to the Department of Internal Medicine of Traditional Chinese Medicine and Pediatrics of Changge Second People's Hospital from May 2019 to May 2021 were selected and divided into a treatment group and a control group with 32 cases in each group according to the random number table method.The control group received comprehensive neural rehabilitation therapy,while the treatment group received Jin's three needles combined with Yizhi Tiansui tuina therapy.Both groups were treated for 1 month as a course of treatment,and after 3 courses of treatment,the clinical efficacy,intellectual level,motor ability,daily living ability,as well as serum levels of neuron-specific enolase(NSE),5-hydroxytryptamine(5-HT),dopamine(DA),and norepinephrine(NE)were evaluated in both groups.Results The total clinical effective rate was 93.75%(30/32)in the treatment group and 75.00%(27/32)in the control group,with a statistically significant difference between the two groups(P<0.05).There was no statistically significant difference in the level of intellectual development between the two groups before treatment(P>0.05),indicating comparability.After treatment,the level of intellectual development increased significantly in both groups,with statistically significant differences compared with the pre-treatment levels within the same group(P<0.05),and the improvement was more significant in the treatment group(P<0.05).There were no statistically significant differences in the scores of the Gross Motor Function Measure(GMFM)and the Activity of Daily Living(ADL)scale between the two groups before treatment(P>0.05),indicating comparability.After treatment,the scores of GMFM and ADL increased significantly in both groups,with statistically significant differences compared with the pre-treatment levels within the same group(P<0.05),and the increase was more significant in the treatment group(P<0.05).There were no statistically significant differences in serum levels of NSE,5-HT,DA,and NE between the two groups before treatment(P>O.05),indicating comparability.After treatment,the serum level of NSE decreased significantly,while the levels of 5-HT,DA,and NE increased significantly in both groups,with statistically significant differences compared with the pre-treatment levels within the same group(P<0.05),and the decrease or increase was more significant in the treatment group(P<0.05).Conclusion Jin's three needles combined with Yizhi Tiansui tuina therapy is effective in the treatment of childhood ID with deficiency of liver and kidney.It can not only significantly improve the intellectual level,motor ability,and daily living ability of children with ID,but also regulate the levels of serum NSE and monoamine neurotransmitters,making it worthy of clinical application and promotion.
