摘要
目的分析体外膈肌起搏器联合综合康复训练在脑卒中后吞咽困难患者中的应用效果。方法选取2022年1月-2023年3月在南华大学附属南华医院接受诊疗的脑卒中后吞咽困难患者100例作为研究对象,遵循随机对照原则分组,对照组50例采用常规康复方案进行治疗,研究组50例采用体外膈肌起搏器联合综合康复训练治疗,比较两组疗效、吸入性肺炎发生情况、吞咽功能、营养状况与生活质量。结果治疗后,研究组总有效率(94.00%)高于对照组(80.00%),研究组患者中吸入性肺炎的发生率(2.00%)低于对照组(14.00%),研究组患者的洼田饮水试验分级优于对照组,且功能性经口摄食量表(FOIS)评分高于对照组,研究组的微型营养评定简表(MNA-SF)及吞咽障碍特异性生活质量量表(SWAL-QOL)评分也高于对照组,以上差异均有统计学意义(均P<0.05)。结论体外膈肌起搏器联合综合康复训练可有效提高脑卒中后吞咽困难患者的疗效,降低吸入性肺炎的发生率,改善吞咽功能,并且促进患者的营养状况与生活质量提高。
Objective To analyze the application effect of extracorporeal diaphragm pacemaker combined with comprehensive rehabilitation training in patients with dysphagia after stroke.Methods 100 cases of patients with dysphagia after stroke who were received treatment in Nanhua Hospital,University of South China from January 2022 to March 2023 were divided into groups according to the principle of randomized control.50 patients in the control group were treated by routine rehabilitation program,and 50 patients in the study group were treated by external diaphragm pacemaker combined with comprehensive rehabilitation training.The effects,incidence of aspiration pneumonia,swallowing function,nutritional status and quality of life of the two groups were compared.Results After treatment,in the study group,the total effective rate(94.00%)was higher than that of the control group(80.00%),the incidence of aspiration pneumonia in the study group(2.00%)was lower than that of the control group(14.00%),and the classification of the lowland drinking water test in the study group was better than that of the control group,and the FOIS score was higher than that of the control group.The scores of MNA-SF and SWAL-QOL in the study group were also higher than those in the control group,with statistical significance(all P<0.05).Conclusion Extracorporeal diaphragm pacemaker combined with comprehensive rehabilitation training can effectively improve the efficacy of patients with dysphagia after stroke,reduce the incidence of aspiration pneumonia,improve swallowing function,as well as promote the improvement of patients'nutritional status and quality of life.
