摘要
目的:探讨全身麻醉与脊髓麻醉应用于日间输尿管镜碎石术老年患者的临床效果。方法:选择2022年2月—2023年9月钟祥市中医院收治的256例日间输尿管镜碎石术老年患者作为研究对象,以随机数表法均分为对照组和研究组,各128例。对照组应用全身麻醉,研究组应用脊髓麻醉。比较两组麻醉相关参数、血流动力学指标及不良反应发生率。结果:研究组麻醉准备时间、手术时间、手术室麻醉相关时间、坐立时间、下床活动时间长于对照组,麻醉苏醒时间、出院时间均短于对照组,差异有统计学意义(P<0.05)。麻醉前、麻醉后40 min、术毕、术后5 min、10 min、20 min时两组血压、心率、血氧饱和度比较,差异无统计学意义(P>0.05);麻醉时、麻醉后1 min、3 min、5 min、10 min、15 min、20 min、30 min,研究组血压、心率、血氧饱和度均高于对照组,差异有统计学意义(P<0.05)。研究组疲劳、排尿困难等不良反应发生率均低于对照组,差异有统计学意义(P<0.05)。结论:与全身麻醉相比,日间输尿管镜碎石术老年患者应用脊髓麻醉苏醒时间和出院时间更短,血流动力学更稳定,疲劳、排尿困难发生风险更低。
Objective:To investigate the clinical effect of general anesthesia and spinal anesthesia in elderly patients undergoing daytime ureteroscopic lithotripsy.Method:A total of 256 elderly patients admitted to Zhongxiang Traditional Chinese Medicine Hospital from February 2022 to September 2023 were selected as the study objects,and they were divided into control group and study group by random number table method,with 128 cases in each group.The control group was given general anesthesia,and the study group was given spinal anesthesia.Anesthesia related parameters,hemodynamic indexes and incidence of adverse reactions were compared between the two groups.Result:The anesthetic preparation time,operation time,anesthesia related time,sitting time and getting out of bed time in the study group were longer than those in the control group,and the anesthesia recovery time and discharge time were shorter than those in the control group,the differences were statistically significant(P<0.05).Before anesthesia,40 min after anesthesia,at completion of operation,and 5 min,10 min and 20 min after operation,there were no significant differences in blood pressure,heart rate and oxygen saturation between the two groups(P>0.05).At anesthesia,1 min,3 min,5 min,10 min,15 min,20 min and 30 min after anesthesia,the blood pressure,heart rate and blood oxygen saturation of the study group were higher than those of the control group,and the differences were statistically significant(P<0.05).The incidence of adverse reactions such as fatigue and dysuria in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Compared with general anesthesia,the recovery time and discharge time of spinal anesthesia in elderly patients with daytime ureteroscopic lithotripsy are shorter,hemodynamic stability is more stable,and the risk of fatigue and dysuria is lower.
