摘要
目的分析2型糖尿病(T2DM)伴多个并发症住院患者相位角(PhA)与肌肉质量减少的相关性及PhA对肌肉质量减少的预测价值。方法回顾性横断面研究。以2023年10月至2024年3月在河南省人民医院住院的356例T2DM伴多个并发症患者为研究对象,收集患者基本情况及血生化分析指标结果。采用Inbody 770多频生物电阻抗(BIA)进行人体成分分析。按照2019年亚洲肌肉减少症工作组推荐的BIA法标准将患者分为肌肉质量减少组与肌肉质量正常组。比较两组体质指数(BMI)、PhA、四肢骨骼肌指数(SMI),Logistic回归分析肌肉质量减少的危险因素,并用受试者工作特征曲线评估PhA对肌肉质量减少的预测价值。结果356例T2DM伴多个并发症成年患者中,肌肉质量减少52例,纳入肌肉质量减少组;肌肉质量正常304例,纳入肌肉质量正常组,肌肉质量减少的检出率为14.61%。肌肉质量减少组BMI、PhA、SMI显著降低。Logistic回归分析显示BMI(OR=0.596,95%CI 0.472~0.753)、PhA(OR=0.384,95%CI 0.163~0.908)与肌肉质量减少的风险呈负相关。BMI、PhA预测肌肉质量减少的受试者工作特征曲线下面积分别为0.705(95%CI 0.594~0.816)、0.827(95%CI 0.749~0.904),两者联合预测的受试者工作特征曲线下面积为0.858(95%CI 0.787~0.929)。结论肌肉质量减少在T2DM伴多个并发症患者中较为普遍,采用BIA法获得的BMI、PhA是T2DM并发症的独立危险因素。PhA、BMI具有较高的肌肉质量减少的预测价值,可作为早期筛查和管理肌肉质量减少的有效指标。
Objective To analyze the correlation between phase angle(PhA)and muscle mass decline in patients with type 2 diabetes mellitus(T2DM)complicated by multiple complications,and to investigate the predictive value of PhA on muscle mass decline.Methods A total of 356 patients diagnosed with T2DM complicated by multiple complications who were admitted to Henan Provincial People’s Hospital from October 2023 to March 2024 were selected as the research objects of the retrospective cross-sectional study.The general information and blood biochemical analysis indicators were collected from the selected patients.Inbody 770 bioelectrical impedance analysis was used for body composition analysis.According to the Asian Working Group for Sarcopenia BIA standard updated in 2019,the selected patients were divided into the muscle mass decline group and the normal muscle mass group.The body mass index(BMI),PhA and limb skeletal muscle index(SMI)were compared between the two groups.Logistic regression analysis was used to analyze the risk factors for muscle mass decline,and receiver operating characteristic curve(ROC)was used to evaluate predictive value of PhA for muscle mass decline.Results Fifty-two of 356(14.61%)adolescent patients with T2DM complicated by multiple complications experienced muscle mass decline;and they were enrolled in the muscle mass decline group.A total of 304 patients had normal muscle mass,and they were allocated into the normal muscle mass group.The detection rate of muscle mass decline was 14.61%.The values of BMI,PhA and SMI in the muscle mass decline group decreased significantly.Results of logistic regression analysis revealed that BMI(OR=0.596,95%CI:0.472-0.753)and PhA(OR=0.384,95%CI 0.163-0.908)were negatively correlated with the risk of muscle mass decline.The area under the receiver operating characteristic curve of two single indicators,BMI and PhA,in predicting muscle mass decline were 0.705(95%CI:0.594-0.816)and 0.827(95%CI:0.749-0.904),respectively,and the area under the receiver operating characteristic curve of the combination of them in prediction was 0.858(95%CI:0.787-0.929).Conclusions Muscle mass decline is common in T2DM patients with multiple complications.BMI and PhA obtained by BIA are independent risk factors of complications of T2DM.PhA and BMI have high predictive value for muscle mass decline,and they can be used as effective indicators for early screening and regulation of muscle mass decline.
