摘要
目的探讨支气管哮喘患者血清抗凝血酶Ⅲ(AntithrombinⅢ,AT⁃Ⅲ)、CD5抗原蛋白(Recombinant CD5 Antigen Like Protein,CD5L)、Toll⁃样受体4(Toll⁃like receptor4,TLR4)与气道炎性反应的相关性。方法选取我院2018年2月至2021年5月收治的98例支气管哮喘患者作为研究对象,根据支气管哮喘发作情况分为急性组(n=49例)和缓解组(n=49例),同期健康体者作为对照组(n=49例),测定三组患者血清AT⁃Ⅲ、CD5L、TLR4水平、气道炎症标志物[转化生长因子⁃β1(Transforming growth factor⁃β1,TGF⁃β1)、白细胞介素⁃13(Interleukin⁃13,IL⁃13)]、肺功能检测,分析血清AT⁃Ⅲ、CD5L、TLR4与气道炎症反应的相关性。结论:急性组血清AT⁃Ⅲ、TLR4表达水平明显高于缓解组和对照组,急性组CD5L表达水平明显低于缓解组和对照组(P<0.05),缓解组血清AT⁃Ⅲ、TLR4表达水平明显高于对照组,缓解组CD5L表达水平明显低于对照组(P<0.05)。急性组患者TGF⁃β1、IL⁃13表达水平明显高于缓解组和对照组,缓解组上述表达水平高于对照组(P<0.05)。急性组患者TGF⁃β1、IL⁃13表达水平明显高于缓解组和对照组,缓解组上述表达水平高于对照组(P<0.05)。急性组FEV1%pred、FEV1/FVC表达水平明显低于缓解组和对照组(P<0.05),缓解组上述表达水平低于对照组(P<0.05)。血清AT⁃Ⅲ、TLR4与TGF⁃β1、IL⁃13呈正相关(r=0.712,0.764,0.704,0.692,P<0.001),血清CD5L与TGF⁃β1、IL⁃13呈负相关(r=-0.779,-0.742,P<0.001)。结果支气管哮喘患者血清AT⁃Ⅲ、TLR4表达水平明显升高,血清CD5L表达水平明显降低,三者表达水平与支气管哮喘患者气道炎症反应存在相关性,可用于支气管哮喘病情严重程度的辅助诊断。
Objective To investigate the correlation between serum antithrombinⅢ(AT⁃Ⅲ),CD5 antigen protein(CD5L),Toll⁃like receptor 4(TLR4)and airway inflammatory response in patients with bronchial asthma.Methods Nine⁃ty⁃eight patients with bronchial asthma admitted to our hospital from February 2018 to May 2021 were selected and divided into an acute group(n=49)and a remission group(n=49)according to bronchial asthma attack conditions,and healthy subjects during the same period were used as a control group(n=49).Serum AT⁃Ⅲ,CD5L,TLR4 levels,airway inflammatory markers[transforming growth factor⁃β1(TGF⁃β1)and interleukin⁃13(IL⁃13)]and lung function were measured in the three groups.The correlation between serum AT⁃Ⅲ,CD5L,TLR4 and airway inflammatory response was analyzed.Results The expression levels of AT⁃Ⅲand TLR4 in the acute group were significantly higher than those in the remission control group,and the expression levels of CD5 in the acute group were significantly lower than those in the remission control group(P<0.05).The expression levels of AT⁃Ⅲand TLR4 in the remission group were significantly higher than those in the control group,and the expression levels of CD5L in the remission group were significantly lower than those in the control group(P<0.05).The expression levels of TGF⁃β1 and IL⁃13 in the acute group were significantly higher than those in the remission control group,and the above expression levels in the remission group were higher than those in the control group(P<0.05).The expression levels of TGF⁃β1 and IL⁃13 in the acute group were significantly higher than those in the remis⁃sion control group,and the above expression levels in the remission group were higher than those in the control group(P<0.05).The expression levels of FEV1%pred and FEV1/FVC in the acute group were significantly lower than those in the remission group and the control group(P<0.05).The above expression levels in the remission group were lower than those in the control group(P<0.05).Serum AT⁃Ⅲand TLR4 were positively correlated with TGF⁃β1 and IL⁃13(r=0.712,0.764,0.704,0.692,P<0.001),while serum CD5L was negatively correlated with TGF⁃β1 and IL⁃13(r=-0.779,-0.742,P<0.001).Conclusion The expression levels of AT⁃Ⅲand TLR4 in serum of patients with bronchial asthma were significantly increased,and the expression levels of CD5L in serum were significantly decreased.Thus,the expression levels of AT⁃Ⅲand TLR4 were correlated with airway inflammation in patients with bronchial asthma,which could be used for the auxiliary diagnosis of the severity of bronchial asthma.
