颗粒脂肪联合纳米脂肪多层次填充矫正泪槽凹陷的效果

摘要 目的探讨颗粒脂肪联合纳米脂肪多层次填充矫正泪槽凹陷的临床效果。方法2020年6月至2021年4月,合肥市第一人民医院整形外科就诊的泪槽凹陷女性患者32例,年龄34~63(48.1±8.8)岁。于下眶缘的骨膜上层泪槽凹陷区分散注射颗粒脂肪,单侧注射0.5~1.5 ml,于泪槽凹陷及皮肤细纹区域皮下多通道连续多点注射纳米脂肪,单侧0.5~0.8 ml。术后1、6个月,以医师判断、第3方医师经Sadick和Bosniak泪槽畸形等级量表评分评估效果,患者行满意度调查。结果术后随访6个月,32例患者中29例术后下睑皮肤外观平整,泪槽畸形得以矫正改善,泪槽沟填充区域较饱满,细纹及下睑皮肤色素改善,睑颊区过度自然,视觉上更加年轻化,未见明显并发症。3例第1次填充术后1个月仍有组织容量缺失,凹陷改善不明显,给予2次填充。32例患者术后满意29例(90.6%)。结论颗粒脂肪联合纳米脂肪填充矫正泪槽凹陷畸形安全有效,手术损伤轻微,术后恢复时间较短,患者满意度较好,值得临床应用。 Objective To investigate the clinical curative effect of autologous granular fat combined with nanofat filling in the treatment of tear trough deformity.Methods From June 2020 to April 2021,32 patients with tear trough deformity were included in the study,aged from 34 to 63(48.1±8.8)yaers.Patients with primary lacrimal groove depression and lacrimal groove more than 6 months after lower eyelid blepharoplasty were selected.Granular fat was injected dispersedly into the lacrimal groove depression area on the upper periosteum of the lower orbital margin,the injection volume on one side ranged from 0.5 ml to 1.5 ml.And nanofat was injected subcutaneously into the area of tear trough deformity and skin fine lines to correct the lacrimal groove depression deformity,the injection volume on one side ranged from 0.5 to 0.8 ml.One month and six months after operation,Sadick and Bosniak tear trough deformity rating scale(TTRS)was used to score the effects in the form of patient′s self-assessment,doctor′s judgment and third-party evaluation.The improvement effect was evaluated,and the clinical effects before and after operation were compared and analyzed.Patient′s satisfaction surveys were also conducted.Results All patients were followed up for 6 months,of which 29 patients had a natural transition between the lower eyelid and cheek,the appearance of lower eyelid skin was smooth,and the tear trough deformity disappeared.Fat filling made the skin in the lacrimal groove area plumper and brightened up the skin tone.The effect was satisfactory without obvious complications.However,three patients felt that one month after the first fat injection filling,there was still tissue volume loss,and the depression improvement was not obvious,and so they were given a second filling.Postoperative satisfaction was reported in 29 out of 32 patients(90.6%).Conclusions The application of granular fat combined with nanofat filling in the correction of tear tough deformity is safe and effective.The patients recover quickly,the surgical injury is slight,and the rate of satisfaction is high.The operation is worth popularizing.
