健康教育对脑梗死患者生活质量及负性心理影响的Meta分析 被引量:2

A meta-analysis on the impact of health education on the quality of life and negative psychology of patients with cerebral infarction

摘要 目的系统性评价健康教育对脑梗死患者生活质量及负性心理的影响,为脑梗死患者健康教育提供循证医学证据。方法在中国知网、万方、维普、Cochrane library、Pubmed、Embase、Web of science数据库检索健康教育对脑梗死患者生活质量及负性心理影响相关文献,时限为建库至2023年8月30日。采用Stata 16.0和Rev 5.3进行统计分析。结果共纳入14篇文献,其中涉及生活质量8篇,负性心理6篇,共1364例脑梗患者。Meta分析结果显示,研究组总体健康状况(WMD=13.11,95%CI:9.05~17.18,P<0.001)、躯体功能(WMD=0.42,95%CI:0.16~0.67,P=0.001)、生理机能(WMD=16.51,95%CI:12.94~20.09,P<0.001)、躯体角色(SWD=1.01,95%CI:0.65~1.38,P<0.001)、情感功能(SWD=0.98,95%CI:0.58~1.38,P<0.001)、社会功能(SWD=1.10,95%CI:0.75~1.45,P<0.001)及精神健康(WMD=9.60,95%CI:6.31~12.88,P<0.001)评分均高于对照组,但两组活力评分间的差异无统计学意义(WMD=13.55,95%CI:-1.31~28.40,P<0.001)。研究组焦虑(SWD=-2.54,95%CI:-3.49~-1.58,P<0.001)和抑郁评分(WMD=-12.84,95%CI:-15.66~-10.02,P<0.001)均低于对照组。结论健康教育有助于改善脑梗死患者的生活质量和负性心理,然而仍需开展高质量的RCT研究来进一步验证及提供高质量证据。 Objective To systematically evaluate the impact of health education on the quality of life and negative psychology of patients with cerebral infarction,to provide evidence-based medical evidence for health education of patients with cerebral infarction.Methods A search was conducted on the database of CNKI,Wanfang,Vip,Cochrane library,Pubmed,Embase and Web of science to retrieve related literatures,the time limit was beginning with database creation through August31,2023.Analysis was performed by using Stata 15.0 and Rev 5.3.Results A total of 14literatures were included,which including 8 literatures about quality of life and 6 papers about negative psychology,the total number of subjects included was 1364.The results showed that experimental group had higher scores on the quality of life in the following categories:Overall health status(WMD=13.11,95%CI:9.05-17.18,P<0.001),somatic functioning(WMD=0.42,95%CI:0.16-0.67,P=0.001),physical function(WMD=16.51,95%CI:12.94-20.09,P<0.001),somatic role(SWD=1.01,95%CI:0.65-1.38,P<0.001),emotional function(SWD=0.98,95%CI:0.58-1.38,P<0.001),social functioning(SWD=1.10,95%CI:0.75-1.45,P<0.001)and mental health(WMD=9.60,95%CI:6.31-12.88,P<0.001).However,the difference between the score of vitality between the two groups was not statistically significant(WMD=13.55,95%CI:-1.31~28.40,P<0.001).The scores of anxiety,(SWD=-2.54,95%CI:-3.49~-1.58,P<0.001)and depression(WMD=-12.84,95%CI:-15.66~-10.02,P<0.001)were both lower in the experimental group than in the control group.Conclusions Health education can help improve the quality of life and negative psychology of patients with cerebral infarction,but highquality RCT studies are needed to further validate and provide high-quality evidence.
