摘要
支气管扩张症是一种气道反复受炎性侵袭的慢性疾病,其发病机制与感染、免疫等因素有关,临床治疗主要以缓解急性发作期症状为主,缺乏对该病整个发病过程的有效管理。中医认为该病属“咳嗽”“咳血”“肺痈”范畴,病机多为本虚标实,病因多为痰、热、淤,应以“清化”为主要治疗原则,同时兼以固护阴液。中药方清金化痰汤源于明代医书《医学统旨》,基于中医深厚的历史文化底蕴,该方具有能清又能润的特点,对支气管扩张症的治疗效果显著,为诸多医家所用。此外,该方还可通过多通路传导、多靶点亲和的方式达到改善气道重构、提高免疫能力、弱化炎性反应、减少气道黏液高分泌的目的,临床价值颇高。该文通过整理近十年来的有关文献,从中医认识、药理机制、临床应用三方面阐述清金化痰汤治疗支气管扩张症的优势,为该病的临床治疗提供新依据。
Bronchiectasis is a chronic disease in which the airways are repeatedly invaded by inflammation.Its pathogenesis is related to infection,immunity and other factors.Clinical treatment mainly focuses on relieving the symptoms of acute attacks,and lacks effective treatment of the entire pathogenesis of the disease.manage.Traditional Chinese medicine believes that this disease belongs to the categories of“cough”,“hemoptysis”and“pulmonary abscess”.The pathogenesis is mostly deficiency in origin and excess in excess.The causes are mostly phlegm,heat and stasis.“Qinghua”should be the main treatment principle,and both Solid protective Yin liquid.The traditional Chinese medicine prescription Qingjin Huatan Decoction originated from the Ming Dynasty medical book“General Purpose of Medicine”.Based on the profound historical and cultural heritage of traditional Chinese medicine,this prescription has the characteristics of clearing and moistening.It has a significant therapeutic effect on bronchiectasis and is used by many doctors.In addition,this prescription can also achieve the purpose of improving airway remodeling,improving immunity,weakening inflammatory reactions,and reducing airway mucus hypersecretion through multi-channel conduction and multi-target affinity,and has high clinical value.This article expounds the advantages of Qingjin Huatan Decoction in the treatment of bronchiectasis from three aspects:traditional Chinese medicine understanding,pharmacological mechanism,and clinical application,by sorting out relevant literature in the past ten years,and provides new basis for the clinical treatment of this disease.
