摘要
肾性蛋白尿之病机本质为风湿瘀之实邪作祟,袭肺碍脾扰肾,而致脏腑功能失调,气机失常,清浊失序,水谷精微不循常道。虽呈谷精外漏之虚象,然其虚非为正虚,乃由邪气困正而成,邪气郁结于内,脏腑功能难复。治疗当注重审证求因以祛邪,邪困得解则脏腑功能复常,清升浊降,精微不泄,以“通”法治“通”疾,看似为“通”,实则为“止”。故秉通因通用为治疗大法,根据不同病邪,分别施用宣散外风、搜剔伏风、淡渗利湿、导利湿热、水血并调、化瘀通络之法。祛邪为要,以通为固,对于病久伤正者,并行通补之法,不使正伤邪进。
The pathogenesis of renal proteinuria is essentially caused by the excess pathogenic factors of rheumatism and stasis,which attack the Lungs,hurt the Spleen,disturb the Kidney,and cause dysfunction of the Zang and Fu organs,dysfunction of the Qi mechanism,disorder of clear matter and turbid matter as well as abnormal movement of the essence transformed from water and food.Although it appears as a deficiency of Essence leaking out,the deficiency is not Zheng Qi deficiency but is induced by Evil factors trapping the Zheng Qi.Evil energy accumulates inside,making it difficult for the Zang and Fu organs to restore the function.When treating the disease,attention should be paid to examining the syndrome and seeking the cause to dispel the Evil factors.If the Evil is resolved,the function of the organs will return to normal,the clear and turbid matter will act normally and the Essence will not leak any more.Using the"Tong(driving)"method in treating the disease functions"Zhi(stopping)".Therefore,"Tong Yin Tong Yong"is commonly used as a treatment method.According to different pathogenic factors,different methods are applied,such as dispersing external Wind,searching and removing latent Wind,Dampness infiltration,Dampness and Heat dredging,simultaneous regulating of Water and blood as well as resolving blood stasis and unblocking collateral.The key is to dispel pathogenic factors,which means strengthening with"Tong"method.For those who suffer from long-term damage to the body,the method of supplementing and"Tong"should be used in parallel to prevent the invasion of pathogenic factors induced Zheng Qi injury.
